Questionnaire Evaluation

This test was designed to evaluate students' knowledge of science at the sixth-grade level. It’s intended purpose was to keep track of how much students understood about
earthquakes, volcanoes, and the rotation and revolution of the Earth. The questions in the survey are designed to determine whether the following outcomes have been reached by
the students. Explain the alterations to Earth's surface brought on by seismic activity and volcanic eruptions. -List the steps to take in anticipation of, during, and recovery from a
natural disaster. Identify the seasons in the Philippine and able to describe them. Explain the differences between rotation and revolution, as well as the consequences of Earth's
revolving around its axis.

To use the properties of materials whether they are useful or harmful. Investigating changes that happen in materials under the following conditions: presence or lack of oxygen and
application of heat. And lastly is to design a product out of local, recyclable solid and/or liquid materials in making useful products. This formative assessment will be used to collect
detailed information that can be used to improve instruction and student learning in teaching Science.
