GROUP H NSTP-2 (SUNDAY 7:30-10:30 AM)

Our group conducted a survey. The purpose of this survey was to find out if respondents have already
attended to a seminar before. To add context and perspective to the answers, we also provided a relevant
question about their Spiritual, Society, Transportation and Communication, Health and Sanitation, Literacy, and
Misdemeanors in their Community.

The first section of the survey is about their family members Spiritual Information. If they are already
baptized by the church or not. And how many of their family members are already married. We also provided a
question if their family members go to mass on Sundays/holidays, If they avail of the sacrament of
Reconciliation, If they receive Jesus Christ in the Blessed Eucharist, and if they already attended to a seminar
before. The second section of the survey is about Socio-Economic Information. We provided a relevant
question about the type of their house, appliances, furniture, and Educational Facilities and if they already
attended to a seminar before. The third section of the survey is about their Transportation and Communication
Information. We provided a relevant question about the availability of the transportation and communication
in their area. The fourth section of the survey is about Health and Sanitation Information. We provided a
relevant question about the health of their family members. The fifth section of the survey is about their
Literacy Information and we provided relevant question about what type do they read and What information /
news do they prefer. The last section of the survey is about Misdemeanors in their Community. We also asked
them for their recommendations.

How did the survey help our group in creating the project? It helps us to provide organizations with
detailed information about the current health state, needs, and concerns in the community. This data may be
used to support the development of a community health improvement plan by demonstrating how and where
resources should be directed to meet community needs.
