I would then conduct a thorough investigation into the causes of the incident,
which may involve reviewing policies and procedures, inspecting equipment, and
analyzing any available data such as CCTV footage.
Based on the findings of the investigation, I would identify any contributing

happening in the future.
Finally, I would communicate the results of the investigation to all relevant parties
