Dear Participant,
Greetings! We are a group of multi-disciplined researchers from the University of Santo Tomas. We are
conducting a study entitled, "Gifted to Give 2021: Lights and Shadows on the Present State of Catholic
Christianity in the Philippines".
You are being invited to participate voluntarily. Before you agree to join, you need to know the important details
of the study, your rights, and responsibilities as a participant, as well as possible risks and benefits so you can
make an informed decision.
This consent form tells you about the study. Please read the information carefully. If you have questions, please
feel free to ask.
PURPOSE AND CONDUCT OF THE STUDY

The Catholic Bishops' Conference of the Philippines (CBCP) has commissioned the University of Santo Tomas
Center for Theology, Religious Studies and Ethics (CTRSE) to conduct a research study about the present
state of Catholic Christianity as a historical account during the 500th anniversary. You have been chosen for
this study because it fulfills the inclusion and exclusion criteria as representatives in the specific categories
namely, the diocesan structure, the consecrated life, and the voices of the laity.
For the Voices of the Laity. Another area covered in this study is the description of the missionary activities of
the lay organizations.
This study explores the life in Christ and in the Church of the Filipino Lay Catholics. Specifically, this study
1. determine the vocation of Filipino Lay Catholics in the Life of the Church.
2. establish the formation of Filipino Lay Catholics.
3. investigate the co-responsibility of Filipino Lay Catholics in the Evangelizing Mission of the
Church
4. determine Filipino Lay Catholics’ vision of the Church in the context of the 500th anniversary of
the arrival of Christianity in the Philippines.
PROCEDURE
The respondents are selected through their Catholic ministry's affiliations purposively. Only those 18 years old
and above who voluntarily accept to be part of the study will be qualified, Roman Catholic, baptized, Filipino
citizen or dual citizen, healthy conditions. Those respondents that are not Filipino citizens, pregnant, with
severe health conditions and infectious diseases will be excluded from the study. The respondents may
anytime withdraw from the study without any consequences.
The researchers will use all efforts to recruit and invite respondents to the study. If the target respondents will
not be met, the margin of error will be adjusted to meet the desired number of volunteered respondents.
You have the right to access your recorded answers and may request a copy of the final research paper
through the contact details of the researcher.

Participation in the study is voluntary and shall not entail any monetary dealings.
BENEFITS
As a historical account – the study will provide a historical account of the "lights and shadows" of the dioceses
and its selected ministries, the consecrated life, the lay organization, and the perception of the laity. This
account will serve as the record of the life of the church during the 500th year anniversary celebration, which
can serve as baseline data for the years to come.
As a basis for Renewal of the Church – The study, according to the Bishops in their Pastoral Letter Celebrating

500th Year of Christianity in the Philippines, hopes to discuss the result of this extensive research work in the
next plenary assembly. With the hope to respond to the call, this study will serve as a sound basis for deciding
as a Conference whether or not it is opportune to call for a Third Plenary Council of the Philippines that would
set our missionary agenda as a Local Church in the next 500 years ahead."
RISKS AND INCONVENIENCES

The researchers will give their utmost effort to ensure that the volunteers are not subjected to danger and that
only unavoidable minor risks are involved. The data will be collected at a time that is convenient for the
participants.
VOLUNTARINESS
This study is completely voluntary, and you will not be pressured or improperly influenced into giving your
informed consent. You have the right to revoke this consent at any moment and for any reason, but you must
notify the Principal Investigator or Researchers. Rest assured that you will continue to be treated in the same
manner as before.
CONFIDENTIALITY
All your information will be kept fully private. Your name will not be revealed unless it is needed by law. Only the
researcher and ethical review committee will have access to your records. Any personal information such as
name, contact number, and email address will remain confidential throughout the research. Your name will be
replaced with a code. The study results may be published in scientific publications, discussed in professional
forums, and may be cited by other researchers. However, no information that identifies the participant will be in
any way publicly reported.
All data collected will be safely stored by the researchers through their personal password-protected computers
and university-established emails. The data gathered will be permanently and irreversibly deleted and disposed
of after one year.

It is important that you follow carefully all the instructions given by the researcher and his/her team regarding
