JOINT CERTIFICATION

We, spouses <Owner’s Name>, hereinafter referred to as the owner of the <Project
Name>, hereinafter referred to as the project, and <Civil Engineer’s Name>,
hereinafter referred to as the supervising civil engineer of the aforementioned project, of
legal ages, Filipinos, and residents of the Republic of the Philippines, after having been
sworn to in accordance with the law, do hereby depose and say:

1.) That we, the owner, to the best of our interest, deliberately made major revisions
to the project against the plan approved by the DBO of Quezon City sometime
after the issuance of the building permit.
2.) That we, the owner, made such revisions simply as a consequence of design
reconsideration transpiring from unexpected circumstances.
3.) That we, the owner, did not make such actions with the intent to circumvent the
policies and regulations, whatsoever, established by the DBO of Quezon City.
4.) That we, the owner, by any means, did not coerce the builder/contractor, the
appointed professionals, or any of their respective subordinates into such
undertaking prior to this event.
5.) That I, the appointed supervising civil engineer, participated in this undertaking
with good intent to serve the better needs of the owner.
6.) That I, the appointed supervising civil engineer, to the best of my abilities,
bestowed professional and truthful expertise in accordance with the law and the
building code of the Philippines.
7.) That both we, the owner, and the appointed supervising civil engineer, willfully
participated in the realization of the said project with full knowledge of its
implications.
