Dr. Ma. Patricia V. Azanza

Officer-in-Charge
College of Home Economics
University of the Philippines Diliman

Dear Dr. Azanza:

Greetings!

The Philippine Association of Nutrition – Alpha Chapter (PAN – Alpha) of the University of
the Philippines Diliman is a duly non-stock, non-profit, College of Home Economics-based
academic organization committed to instilling professional growth and excellence in the field of
nutrition among its members. For years, the organization has engaged itself in activities which aim
to promote nutrition consciousness among Filipinos, and to disseminate the latest information on
nutrition.

This November, in celebration of World Diabetes Day, PAN-Alpha is planning to conduct a

Nutrition Clinic (NutriClinic) entitled “NutriClinic 2019: DIET-BEAT-THIS! Preventing
Diabetes Through Proper Diet and Nutrition.” This is a three-day event which aims to provide
free anthropometric measurements and consultations for the students of the College of Home
Economics, and to serve as an avenue to spread nutrition knowledge to the students of the
College of Home Economics on diabetes and how to prevent this among them. We wish to
conduct the event on November 20-22, 2019 from 8:30 AM to 5:30 PM in the Alonso Hall,
Lobby.

In this regard, we would like to ask your office to permit us to conduct the activity. Attached
herewith is the project proposal for your perusal. Should you have any questions, please do not
hesitate to contact Ms. Francesca Raphaella Q. Malubay at 0915 615 3552 or
fqmalubay2@up.edu.ph.

Thank you very much and we are hoping for your most favorable response.

Respectfully yours,

Francesca Raphaella Q. Malubay Anne Nicole S. Gonzales
