Greetings!

I am Janie-Lyn Prieto-Broñola, a JHS English teacher at Talisay City National High

School and is currently enrolled for Thesis 1 at Cebu Normal University.

A few weeks ago, I sent you a request for validating two of the research tools to be used
in my study. However, as advised by my mentor, only one will be needed.

In this light, I would like to ask your expertise again to re-validate the questionnaire
attached. This tool will be used for my interview of the participants.

The study is entitled ‘COMMUNICATIVE ACTIVITIES IN STUDENT'S SELF-

LEARNING MODULES: ANALYSIS AND STUDENT PERCEPTION’. The purpose of
this study is to narrate and ascribe the experiences and perceptions of the students in
doing the communicative tasks in the English modules. Their responses may contribute
to the preparation, development and evaluation of modules.

I hope that you will again share your expertise on this field. I will also be very glad to
