First Quarter Performance Task: Mental Wellness Plan


Target Date of Submission: Oct 7, 2022

Performance Task Scenario: The community faces different kinds of social issues. Identify one
that personally affects you and/or you strongly advocate for (see to it that the social issue you
choose is also based in your assigned JEEPGY group in CLF). In order for us to maintain our
stamina in addressing these problems, it is highly necessary to keep our well-being in check.
Hence, you are going to come up with a Personalized Mental Health Plan which aims to guide
you in nourishing your mental, emotional, cognitive and physical health amidst the crisis we
face. In order to accomplish this, you shall reflect and answer the given guide questions and
mental health plan template.

Culminating Performance Standard:  The learner will be able to draw out the connections of
the aspects of their development to a social context or issue through an introspection and
demonstrate their introspection in a mental health plan which includes their personal ways to
cope with stress and maintain mental health.

Mechanics:

1. Anchoring on the social issue from your CLF 1st Quarter performance task, you are going
to individually reflect on and answer the given template which includes the Social Issue
Reflection and Mental Wellness Plan. (See Annex A for the template).
2. The final output shall be in IMAGE (.jpeg/.jpg/.png) Format. Submit your accomplished
mental wellness plan in Quipper Essay, entitled, PerDev_1st Quarter PT_Mental
Wellness Plan. For outputs with multiple pages, you may attach a link that contains the
images.

II. Personal Wellness Plan


1. You have done your personalized “BASIC Ph Coping Shield” as one of your Performance
checks in Module 3. Insert your “BASIC Ph Coping Shield” in this first portion of your
Mental Wellness Plan.
2. Based on the social issue you have shared, you are going to craft your personalized
Wellness Plan using the ways you personally cope. In this way you have a guide on how
to protect and take care of yourself amidst the social issues collectively faced.
Your Personal Wellness Plan should be S.M.A.R.T (Specific, Measurable, Attainable,
Relevant and Time bound).
Specific- Your plan should point out specific tasks you shall do to take care of
yourself.
Measurable- Adding quantifiable or measurable criteria to your goal will allow
you to measure progress as you work towards achieving your goal.
Attainable- You have to make sure that your Mental Health Plan is achievable
and realistic; consider the resources that are available to you.
Relevant- Your plan should be relevant to the social issue you have reflected
upon.
Time bound- There should be a clear timetable as to when you plan to do the
