What's in the form:

Basic details of the performance of SDOs as delivery

units. These include the OPCRF Rating for FY 2020,
average OPCRF ratings of schools within the SDO,
budget utilization rate, % of liquidation, and
compliance to Good Governance Conditions.
How to fill up the form:

1. Fill in the data required per schools division offices.

Please don't edit or touch cells with formulas.
2. Sort total scores from highest to lowest. In case of a
tie, SDOs with the same total scores shall be ranked
based on the improvement from previous year’s
performance in OPCRF overall score, BUR, %
liquidation of Cash Advances, and average OPCRF
ratings of schools, in that order.
3. Out of the total number of SDOs, follow the
prescribed distribution below in identifying the
ranking.
BEST Delivery units : top 10%
BETTER Delivery units : next 25%
GOOD Delivery units : next 65 %
4. Once completed, save the file as "SDO LEVEL_FORM
1.3_REGION________"
5. Send soft copy to the CO PMT Secretariat for
validation before disseminating to SDOs and schools.
