We, the surviving legal Heirs /Children and Beneficiaries of our late mother, Rosalinda P. Matienzo,
would like to appeal and request to the Board of Pasig Cooperative to help us claim the 100% amount
left by our mother on her passbook account and we hope the Coop to release the whole amount to the
declared Beneficiaries indicated on our mother’s contract of account. Our mother cared and kept her
Passbook active to be used in time of her demease. We expected the beneficiaries to receive the
whole amount to be used in the settlement of our mother’s burial expenses. We don’t have enough
money to pay for all the burial expenses including the Columbarium vault we loaned. The cash fund on
our mother’s passbook account was intended in helping us survive on the expenses we had.

We, as one therefore, appeal and requesting the respective Board to help us claim and receive the
100% that was left by our mother on her Coop Passbook Account and to release the whole amount to
her declared beneficiaries, Maria belen M. Sacro & Vera Marie M. Santos for us to have enough funds
to pay the unsettled burial expenses we had.

In this regard, We are hoping for your understanding and kind consideration on the matter.
