Re : Request to Expedite the Reconsideration of Tax Liabilities

Greetings!

We, Angie G. Cabardo, 27 y/o, Mia Precious G. Cabardo, 22 y/o and Althea Quile Mea Gumban, 9 y/o,
the daughters of the late Vivian Calvo Gumban, would like to request to expedite the application for
reconsideration of the tax liabilities that we applied last November 21, 2017 of our mother who passed
away last November 8, 2017, battling stage 4 ovarian cancer for 2 ½ years.

In this connection, we are asking your good office to allow us to pay the 10% of the total amount,
amounting to Eighty Two Thousand Five Hundred Three Pesos and 79/100 (Php82, 503.79) and the
remaining will be free from settlement so that we can used for other obligations/debts being used for
medication and hospitalization of our deceased mother.

Early 2015, our mother is diagnosed with malignant ovarian cancer stage 4 and since then, she often
admitted to the hospital until she passes away last November 8, 2017. She is separated to our father for
16 years and she is the only bread winner to our family. Since then, I Angie Cabardo as an OFW
supported my two younger sisters and their studies. Unfortunately this year my employment contract in
Kuwait got cancelled and I don’t know what to do anymore to support my younger sisters.

We also request to lift the hold order being imposed to the bank account of our late mother Vivian
Calvo Gumban and to the service vehicle and our family home so that we can use it in our daily life as
starting source of living.
