for own nursing decisions, the “patient” states. Also, for drug administration, I practice the 7 rights and 3 checks

1. Bases nursing practice on 1 All nursing practice should be current evidenced base, in lab we have practiced
current evidence from nursing doing assessments specific to the client’s condition and/or medications they are on.

humanities assessment to listen to their lungs, C.W.M.S for possible edema to extremities
especially to their lower.
2. Knows how + where to access 1 For our courses, I access the student guide and the reading guide for support, then
information to support + for nursing practice such as scope of practice I would access BCCNM for support.
provide safe, competent +
ethical nursing practice + care
for clients
3. Uses critical thinking when 3 Throughout the program we have practice creating care plans in almost all our
collecting + interpreting data, courses, critical thinking helps us create a better and well-structured care plan that
planning, implementing + will benefit the client. For example, if a client’s diagnosis is unstable angina, us
evaluating nursing care nurses will not plan to treat the pain, but think about the client safety and possible
education that could be taught to the client, so that they know how they could
manage it if ever there is no help around.
4. Collects information on client 2 I’ve done assessments on client based on the scenario given and would record my
status + care needs from a findings and chart them. I would then write a care plan based on my findings and
variety of sources using discuss this with my classmates to see if they might’ve done something different and
assessment skills + a review of why they did that way.
pertinent clinical data
5. Identifies, analyzes + uses 2 In one of the scenarios, where the client had difficulty swallowing or had a choking
relevant decision support tools episode. I decided to refer them to a dietician for re-assessment of food texture and
+ data when making decisions fluid consistency, teach them “chin-tuck”, the client may also need assistance with
about client status + care feeding and this needs to be communicated with the other health care staff as well as
requirements if there were any dietary changes.
6. Documents client assessments, 2 The end goal for patient care plan should always be positive patient outcome. Care
care needs, planned plans are specific to the patient needs, proper documentation will lead to continuity
interventions + outcomes in a of care which will benefit the patient’s quality of life. This is also a great way to
timely manner advocate for a change the patient may need. With the right documentation, the
communication regarding the patient’s needs with the health care team will be more
efficient and everybody will be on the same page.
7. Communicates client status to 2 Communication is the key to continuity of care. In lab, we have practice charting,
other members of the health which included what care was done, any assessments that was done on the client,
care team as appropriate any concern, and any abnormal findings.
8. Evaluates client responses to 2 After doing my QPA and client states that he/she is in pain, I would do a pain
care + revises the plan of care assessment then provide something that could relieve the pain. When the pain has
as necessary relieved, I would then continue with their care.
9. Responds + adapts to changes 2 Since we have started the program there have been changes to schedule, class hours,
in the practice environment classroom location. With all the changes that has happened I have been adaptable
and worked with the changes.
10. Shares nursing knowledge with 3 I have done most of the skills we are doing in lab at my previous school, which I’ve
clients, colleagues, students + gotten comfortable doing already. Sometimes I just have a hard time-sharing ideas
others and thoughts to other people.

in interactions with clients,
colleagues, students + others

Standard 3: Client-Focused Provision of Service

The licensed practical nurse provides nursing services + works with others in the best interest of clients.
Indicator Eval Comments/Supporting Evidence
1. Makes the client the primary focus 1 When providing care to the client it only makes sense to make them the primary
when providing nursing care focus. I will ask the client if they have any preferences, how they use to do it when
they were able to do it by themselves, any routine, and it is very important to
always involve the client with their care.
2. Involves clients in identifying + 2 Clients should always be involved with their care. To make a goal successful, the
prioritizing their own health goals client should understand the reasoning and possible outcomes for the health goals
+ learning needs set for them. This could also be a teaching opportunity for nurses, to help their
clients understand the situation, and what they can do to achieve the goal. In lab,
when we are given scenarios, it involves client teaching, I would list or discuss
with my partner different teaching I would do and why.
3. Supports clients to learn about the 2 When writing a care plan, there were times where I put that the client would be
health care system + accessing referred to OT/PT, dietician, and social worker. This shows them the health
appropriate health care services services they can access, then I would give them the reasoning behind it. For
example, refer to OT/PT for bar rails in bathroom for safety purposes, and to avoid
much worse situation.
4. Understands + communicates the 2 It is important for us nurses to know the resources our clients can access and the
contribution of nursing to the benefits and support they can get from those sources. For example, the dietician
health of clients could help them with a nutritional plan so that they are getting the right amount of
nutrition in their body.
5. Communicates, collaborates + 2 In lab, I would communicate with the client about the care I’m about to perform. If
consults with clients + other anything seems concerning, I will ask the client about it or discuss it with my
members of the health care team classmates. I will also ask the client if they have any preferences with their care, or
about client care if they have any concerns.
6. Coordinates + facilitates 2 Writing an organized and well-informed documentation or charting will provide
continuity of care services for the continuity of care for the client. For example, if there were any changes in care like
client the client is now on a repositioning schedule q2h, to continue this and to prevent
worsening of pressure sores, health care staffs should be informed.
7. Supervises, leads + assigns 3 In class, we have worked in groups where we would gather information and share
appropriately to other members of to the class. Sometimes we would assign people to record and someone to share
the health care team with the class.
8. Supports + guides other members 2 In lab, when we are brainstorming to write a client’s care plan, I would give
of the health care team to meet suggestion to improve or meet client care needs such as, if they are a fall risk at a
client care needs long-term care and mobile, they should have a commode by their bedside to avoid
walking long distance. Having a night light and a bed alarm during the night if
they get up frequently.
9. Participates in + advocates for 2 When we are writing a care plan, and for example the client has pressure sores
changes that improve client care + developing. I would then put them on a repositioning schedule like turn q2h or
nursing practice q4h, as well as putting barrier cream on pressure points, frequent brief change if
they are heavy soaker, and push fluid to maintain hydration.
10. Recognizes + reports the 5 I have not seen impaired practice within the class.
incompetent or impaired practice
or unethical conduct of another
health professional to the

at all times treat everybody with respect.
2. Represents self clearly + 2 Upon entering the client’s room, I would say, “Good morning Mr. Potter, I’m
accurately with respect to name, Ciena and I will be your nurse today. I came in to check on you and do your vitals
title + role as well as an assessment”. This way the client knows my name, title, role, and
what I’m going to do.
3. Respects + protects client worth, 1 Demonstrates this by providing a non-judgmental environment when providing
dignity, uniqueness + diversity care, asks the client if the care I’m about to proceed with is okay with them, asks
the client’s comfortability level with care, and tells the client what I’m about to do
for example, performing a head-to-toe assessment, before touching the client ask
permission as some clients are uncomfortable when we get into their personal
space.
4. Protects client information + 1 When writing care plans and charting I use the client’s initial instead of their full
maintains privacy + name to protect client’s information. This is a good practice before going to
confidentiality practicum where we’ll be dealing with real clients.
5. Recognizes, respects + promotes 2 When changes are made to the client’s care, it is very important to get them
the client’s right to be informed involved and informed with these changes as they may disagree with some of it.
+ make informed choices We also must respect the client decision. We should discuss the changes to the
client and provide them with education why these changes are put in place, if they
disagree, ask them why. It is the client’s health and if we must make changes they
should know as it is their health.
6. Begins, maintains + ends nurse- 1 By having a professional boundary between the client and I, it will be easy to put
client relationships in a way that their needs first. Listening to them and building a bond where the client could trust

values, beliefs, + experiences after by their families and now working at a facility where I look after other
when providing nursing care people’s parents and/or grandparents was very shocking for me. When I care for
other people, I always have in my mind that the care I will give them will be the
care I want my grandparents to receive. Sometimes this affects my job emotionally
and mentally because I get attached to them.
8. Identifies ethical issues, 5 N/A. I have not encountered ethical issues in the program.
