1. Reference: SWOT Analysis and Risk Register of ORIDMD dated

August 12, 2022, and December 2, 2022.

2. This has reference to the incoming 3 rd Party ISO Certification

scheduled tentatively in the 1 st Quarter of 2023 wherein it has been observed that
there is a recurring Weaknesses and Risk Register of this Office from August to
December 2022 which is the depleted number of personnel.

3. In this regard, may we respectfully request additional personnel to

address this recurring problem encountered in preparation for this coming 3 rd Party
ISO Certification.
