We, the undersigned incorporators, all of legal age voluntarily agreed to form a
religious society under the laws of the Republic of the Philippines.
(INDEPENDENT)
Third: That two-third(2/3) of its membership have given written consent and
voted to incorporate during a meeting of the society.
Fourth: That the incorporation of this religious society is not forbidden by the
competent authority or by the Constitution, rules, regulations or discipline of the
religious denomination, sect or church to which it forms part.
That the purpose of this religious society is for the administration of its affairs,
properties and temporalities.
Sixth: That the religious society shall have its principal office at GOSPEL
CATALYST CHURCH COASTAL GREEN STREET P.F. ESPIRITU I
(PANAPAAN) , BACOOR CITY, CAVITE, REGION IV-A (CALABARZON), 4102;
Seventh: That the number of trustees of the religious society shall be five (5)
and the names, nationalities and residences of the first trustees are as follows:
Ninth: That the level of administrative expenses of the corporation shall not, on
an annual basis, exceed thirty percent (30%) of the total donations received and total
expenses for the taxable year.
Tenth: That in the event of dissolution, the assets of the corporation shall be
distributed to another corporation, organization or association whose
purpose/purposes are similar to the purpose/purposes of the dissolved corporation,
or to the State for public purpose/purposes, or shall be distributed in accordance
with an Order issued by a court of competent jurisdiction.

Revised Corporation Code, and that the willful certification of a report required
under the Revised Corporation Code, knowing that the same contains incomplete,
inaccurate, false, or misleading information or statements, is likewise punishable
under Section 162 of the Revised Corporation Code; and
