It has come to our notice that you have failed to report for work since [Date]. You failed to neither seek
approval nor advise your Immediate Superior of your whereabouts.

Please be reminded that based on our Attendance Policy stipulated in our Company’s Employees Code
of Conduct, unauthorized and unexcused absence from work, including unauthorized extended leave
shall be considered as absence without official leave (AWOL) and warrants corrective actions from
Management.

With this, you are hereby given five (5) days upon receipt of this letter to report to your Immediate
Superior, [Name], and to explain in writing why no corrective action should be imposed on you based on
the violation indicated above. Failure on your part to submit written explanation on the given time
frame shall be construed as a waiver of your right to be heard and we will consider your action as
abandonment of work. The management reserves the right to hold you liable for any loss or damages to
the company or to its client because of your absences.
