role in the development of vascular erectile dysfunction. The examination of tunica
albuginea functional properties is very important in erectile dysfunction management
because it provides the restriction function of venous outflow. The significant non-
invasive methods of the tunica albuginea evaluation are absent.
The researches show that tunica albuginea insufficiency is the direct cause of
venoocclusive incurable erectile dysfunction. It needs surgical intervention. Data of
investigations show the importance of the intracavernous block disorders of as peculiar
“point of no return” in the development of the vascular erectile dysfunction.
Tunica albuginea has been examined by share wave elastography in 5 men with normal
erection and cavernous hemodynamic. The International Index of Erectile Function
(IIED) questionnaire and dopplerography of cavernous vessels with pharmstimulation
by 10 meg PgE1 with isolated examination of tunica albuginea elastic properties have
been carried out.
The results showed significant increase of the tunica albuginea stiffness during erection.
The average value of the Young’s modulus is M¼22,5 kPa, error of average e m¼2,7,
standard deviation ¼6,0 at rest against, so M¼126,2, n¼6,67 and SD¼14,93 in
erection on the right cavernous. The same indexes are on the left cavernous: at rest
M¼21,36, n¼3,39 and SD ¼ 7,59 and M¼136,4, n¼3,67 and SD¼8,22. The re-
sults have been evaluated by Medstat program, that showed high levels of differences
(p0,001).
These results show the dynamic role of tunica albuginea mechanical properties in
providing erections and future possibility of share wave elastography for “point”
diagnostic of its functional conditions and connection with collagenopathy.
We assume that elastography is essential in the evaluation of erections and facilitates
the development of effective methods of conservative treatment of erectile dysfunction
in future.

COMPREHENSIVE TREATMENT OF MALE
PSYCHOGENIC ERECTILE DYSFUNCTION BY SEX
THERAPY ADN PSYCHOTROPIC DRUGS (WITHOUT
PDE5 INH.)

The aim of this presentation is to explain the curative treatment of male psy-
chogenic erectile dysfunction (ED) by sex therapy and psychotropic drugs
(without PDE5 inh). This reduces the PDE5 dependency and non-responsiveness
later on.
Psychogenic ED is very common in conservative Muslim society like Pakistan
where masturbation and sex education is a taboo. Masturbation is religiously
inhibited.
415 patients of erectile dysfunction were treated at Nasim Fertility Center
Lahore, ISD and FSD during Oct. 2015 to Oct. 2016. In these patients 67% are
bellow 40 years age and 84% had the symptoms of less erection or no erection
and less ejaculatory time (duration of the symptoms varies from few months to
many years) and had predominantly psychogenic erectile dysfunction without any
organic cause. This is almost the reverse ratio to European and American studies.
Diagnostic work up included, detailed sexual and medical history, and relevant
lab investigation.
Psychological factors that predispose, precipitate and maintain psychogenic erectile
dysfunction are treated by psychosexual therapy (sex Therapy), which integrates
cognitive behavioral interventions, system/couple interventions and sometimes-
psychoanalytic interventions. Performance anxiety or acute stress in psychogenic ED is
treated by SSRI, benzodiazepines, supported by yohimbine and testosterone and some
time by I/C injection. This pharmacological intervention facilitates early recovery.
Aphrodisiac food and regular physical activity is advised.
Normal Sexual functions are restored in 10 weeks of weekly sessions of sex therapy and
pharmacotherapy.
In conclusion, in male psychogenic erectile dysfunction normal sexual function is
restored by sex therapy, counseling and psychotropic drugs (SSRI, Benzodiazepines).
This reduces PDE5 inhibitor dependence and later on non-responsiveness to PDE5
Inh.

SCHEMA MODES IN SEXUAL OFFENDING

Introduction and Objective: Research on motivation of sex offenders suggests that
prior to committing the offense, offenders go through frustration originating from
unfulfilling their biological and psychological needs. This in turn activates schema
modes defined as changing from moment-to-moment emotional states and coping
responses (Young, 2003). In this context the poster presents preliminary findings from
research that aims to evaluate active schema modes occurring among men accused of
committing sexual crimes.
Methods: To identify schema modes 20 case files of men accused of committing sexual
offenses (10 rapists and 10 child molesters) were analyzed by two independent experts
utilizing the Mode Observation Scale (Bernstein, de Vos, Van den Broek, 2009).
Results: Similarities as well as differences occurred in type and intensity of schema
modes between rapists and child molesters. In both groups the score for child domain
mode was higher during the events prior to the crime, than during the criminal act
itself. It was also observed that the intensity of the modes was higher among rapists.
During the criminal act, the modes among child molesters were more specific than the
modes among rapists, among whom the dominating modes were overcompensation
and undisciplined, angry child.
Conclusions: Criminal case files can be very valuable source of information about
active schema modes and constitute an alternative way of identifying the modes. Their
systematic analysis led to results that present a promising perspective of utilizing the
concept of schema modes in understanding background motivation of sexual offenses
and solidifying this concept as a valuable element of psychological and forensic
assessment and intervention.
Research limitations:
1. Small sample of the preliminary study e further, scaled research requires to be
conducted.
2. Case files as an indirect source of information.
3. The schema mode evaluation may depend on the reliability, solidity, accuracy of the
case files.
