 The client skips his meals and he does not take any vitamins. He gets tired easily. He puts
pillows under his head and back to be able to breathe easier because it’s easier for him to
breathe when seated.

General Appearance

 He looks very weak and tired.

 Client flakes was observed.

 The client's nail bed is yellowish in color. He has convex 160 but long and dirty finger
nails.

Eyes

 His conjunctiva is pale.

Mouth and Oropharynx

 He has pale symmetrical lips.

Abdomen

 His bladder is not distended.

Upper and Lower Extremities

 Muscle tone is poor.
