PIVOT… EMBRACE TECHNOLOGY (PET) PROJECT

COMMITMENT OF UNDERTAKING
I, ______________________________________________, Filipino Citizen and a
resident of ________________________________________________________,
hereby acknowledge that I voluntarily participate, and I understand that the information I
provided relates to my intent in availing the training under DTI Pivot…Embrace
Technology Project and the information that will be collected, processed, protected,
shared, and retained DTI will be used for its pursuits of legitimate purposes, specifically
for the online training and marketing campaigns. Thus, hereby allow DTI - to collect,
use and share personal data for its pursuits of legitimate interests as scholarship grant
provider in accordance to RA 10173 or the Data Privacy Act of 2012.

I further manifest and declare that I will actively participate by finishing the course
packages within six months. I adhere to the rules and regulations of the upskilling
scholarship program and hereby undertake and commit the following:

1. Authorize DTI to use my personal information such as my name, picture,

testimonials in any promotional activity relating to the scholarship programs;
2. Attend the orientation of scholars before the conduct of the training program;

3. Maximize this training program and explore the courses and resources on the
platform;
4. Maintain integrity by keeping my credentials confidential;
5. Keep my account ACTIVE. Should I fail to open my account within 7 days, my
access will be revoked and be given to waitlisted participants;
6. Comply with instructions given by the administrators;
7. Undergo the mandatory assessment as applicable;
8. Attend the graduation ceremonies and other Scholar/DTI activities;
9. Actively participate should DTI conduct a tracer study on the participants of the
said project; and
10. Authorize DTI to use my name, email address and summary of completed
courses to DTI ECO Partners for possible employment at the end of the training
program;
11. Utilize all the skills obtained as practicable as possible and inform the DTI of
updates and results such as employment, career advancement, etc.

This consent and authorization remain valid and subsisting for a limited period
consistent with purposes or until otherwise revoked or canceled in writing.
