in a online and onsite review in FH FATIMA REVIEW CENTER.

In consideration of the review center permitting me to participate in this activity, I hereby agree to the following terms and conditions for my participation:

1. I am an adult, 18 years of age or older, and my participation is elective and voluntary.


2. I attest that I am physically and mentally fit to participate and that I do not have any medical condition that could be aggravated by my participation in this
activity.
3. I am aware of the guidelines connected with attending this review. I agree to not hold accountable or bring legal action against the center, their admin,
instructors and trainers, and marketing staff.
4. This waiver releases the center from all liability relating to injuries that may occur on location before, during, or after the online and onsite class.
5. I acknowledge the risk involved with this activity in actual demo including, but not limited to, muscle tears, sprains, and other physical injuries. I certify
that my participation is voluntary and I have been made aware of the risks. Additionally, I do not have any conditions that may increase my likelihood of
injury.
6. I will make every effort to obey the institution personnel, all safety rules, and will ask for clarification if needed.

I have read this agreement and fully understand its terms. I understand I have given up substantial rights by signing this agreement, and sign it freely and
voluntarily without any inducement.

PAYMENT POLICY

I,________________(NAME),residing at _____________________(ADDRESS) have independently registered starting _________(DATE) in order to participate


in a online and onsite review in FH FATIMA REVIEW CENTER.

In consideration of the review center permitting me to participate in this activity, I hereby agree to the following terms and conditions for my participation:

1. I agreed to pay all of the fees, may be paid in installments, according to the conditions set out below providing that the installment plan is in place before
the initial due date.
2. In the event that all or total balance due is NOT PAID before the PRE-ASSESSMENT date, I am aware that I cannot participate in pre-assessment exam
and actual demo, hence, can only join once fully paid and will be scheduled for the next batch.
3. If payment is not made within 7 days after the indicated date in promissory note, the student will not be accepted at pre-assessment exam and actual
demo class.
4. Failure to pay will result in the revoking of school privileges including but not limited to, the participation of pre-assessment exam and actual demo,
issuance of certificates, the participation in the graduation ceremony and the awarding of Diplomas.

Online class and
group chat policy
GENERAL GUIDELINES FOR STUDENTS ONLINE CLASS

For best online learning experience, FH FATIMA REVIEW CENTER inform all students to abide by the following rules and regulations to maintain and establish proper class proceedings:

1. Observe Punctuality. Every student is expected to show up and be prepared for the class at the exact given time. The grace period is 15 minutes, whoever tries to attempt to enter the Google
classroom beyond the grace period will be declined. Students must be able to accomplish the attendance form before the commencement of each session.

2. Privacy Policy. Everything that will be discussed during the whole class session must remain under the confidentiality policy of FHF Review Center and its students. Taking a screenshot
throughout the time of the class, recording the whole or any session using any device and posting on social media about personal information of students and instructors and other issues that may
reflect bad reputation in general is strictly prohibited.

Disclaimer: If there is an instance, even an attempt, to destroy the reputation of the review center and its employees and students , the institution has the right to immediately FORFEIT the enrollment
and class of the student WITHOUT ANY REFUND as part of moral damage and threat to privacy and professionalism.

3. Professional Conduct. One should always use professional and proper manners when participating in online classes. There is no required attire, but it should not be vulgar. Any offensive
words must NEVER be used.

- If you have any question in mind that you want to ask the instructor, kindly use the chat box to type in your questions and concerns unless the instructor wants you to unmute your
microphone.

- If you want to excuse yourself for a moment, please do so by making use of the chat box.

4. Internet Connection. In order for the online class to run steadily, the student must secure a strong and stable internet connection before the session starts. This is to ensure that during the
course, the instructor and student will have better communication and technical interference can be avoided.

5. Listen Carefully. It is fundamental that all students must optimize the online tutorial by being attentive and productive during the course of the class. Students should bring their focus into
studying and if it is possible, jotting down notes and heeding the tips given by the instructor will be an effective way of learning.

6. ATTENDANCE IS A MUST. Any absences or tardiness (No Show Up or Failed to inform Admin and fill out the attendance for 24 hours will be considered as ABSENT. Please be advised that if
you missed a certain class schedule, there will be NO make-up classes.
FACEBOOK GROUP CHAT POLICY

For professional and effective communication via online, FH FATIMA REVIEW CENTER inform all students to abide by the following rules and regulations to maintain and establish good
relationship with everyone:

1. Make sure your SCREEN NAME and avatar are suitable and appropriate. Any inappropriate or scandalous nickname or photo is prohibited.

2. Treat everyone with respect. Start every conversation with a Greeting. Be polite with FHF personnel and your classmates. Bullying and any kind of offensive words is prohibited.

3. Be Clear and Concise. Be straightforward and use appropriate words to avoid misunderstandings.

4. Respect Status Availability. Do not demand time from others, most people are busy with something and be patient while waiting for a reply.

5. Stay on Topic. Any personal and issues not related to the session will not be tolerated. Avoid personal topics and not necessary for the group.

DON’Ts

1. Don’t Give Out Sensitive Information. Be cautious about the information you share. Giving out information, RECEIPTS, passwords, addresses, and even bank account/credit card
numbers is NOT safe.

2. Avoid Private Conversations in a Group Chat. It’s one thing to directly message people individually if you want to talk about things.

3. Don’t Text Bomb. No flooding of messages. Refrain From Overusing Emojis.

4. Control sharing personal business ads. If you are selling something, make sure to ask permission from the admin before sharing your products and services to group chat.

5. Never Leave a Group Chat Unexpectedly. Let people know that you’re leaving politely.and accepted FHF Guidelines.
