SERVICE AGREEMENT

I. THE PARTIES. This Agreement (“Agreement”) made ________________________, 20____


is by and between FH FATIMA MANPOWER SERVICES., with business address Blk 13 lot 16
Dal;y Village Lontoc Timalan Naic, Cavite, Philippines and hereinafter referred to as the
“AGENCY” and ______________________(Client Name), with residence address at
__________________________________ (address), and hereinafter to as the “CLIENT”
II. TERM OF AGREEMENT. The term of this agreement will start on the Effective Date, and will
on an as-needed basis until the Agreement is terminated by either party, as provided hereunder

FATIMA MANPOWER SERVICES and ___________________________________.
.III. SERVICES REQUESTED. We will provide the services (“Services”) requested and agree
what is agreed upon. The preferred day, time and duration of services will be mutually agreed
upon by you and/or your representative and the agency.
IV. RATES AND FEES

V. DEPOSITS. A deposit equivalent to ______________charge will be expected upon execution
of this contract before the start of services. The agreed total deposit is ______________.The
deposit will be held by the agency without interest for the duration of services.
VI. BILLING. The employee will fill out a timesheet daily. At the end of the employee’s work
week (Monday to Sunday), you will be expected to sign the timesheet as acceptance of the
hours service delivered. Please sign it promptly so the employee can be paid promptly. After the
start of services, invoices will be sent bi-weekly after completion of each service period. Any
questions regarding timesheets or your invoice should be directed to our office.
VII. PAYMENT AND OVERDUE ACCOUNT. Fees for services rendered are payable upon
receipt of invoice. Payment may be made by check, money order or credit card. It is FH FATIMA
MANPOWER SERVICES’ policy not to accept checks endorsed over to the agency. All
payments must be remitted to the address noted above; direct workers are not permitted to
accept payment. An account is considered overdue if not paid within 10 days of the billing date.
Interest will be charged.
VIII. CANCELLATIONS. Cancellations may be made up to 24 hours in advance of a scheduled
visit without charge. We reserve the right to charge for a scheduled visit if insufficient notice is
not given. In the event that a referred worker fails to arrive at the care recipient’s home, we will
make every effort to find a replacement as quickly as possible. If a replacement is not found or if
the worker alters the predetermined weekly schedule in some way, we will adjust the amount
that you are billed accordingly.
IX. CONFIDENTIALITY. The Client shall treat as confidential all information relating to the
Agency’s business and operations and undertake not to disclose to any person outside the
Agency any confidential knowledge of information regarding, the Agency or any of its clients or
the nature of Business.
1. The Client shall not make use of, divulge communicate or disclose to any other similar
organization , person or persons, and the Press and Media or use for any purposes
rather that on the behalf of employer any information concerning the business, accounts
finances, methods, clients, secret dealings or other transaction that is affiliated or in the
same business as the Employer.
2. The Client understands that no information about his/her location, plans for the day,
pictures of the client or family members, associates, or friends shall be shared on any
social media network. He or she will be required to not inform strangers or third (3rd)
parties where he or she shall be spending their time during the day unless the client
grants consent. Recipient may only grant such consent if he or she is able to make
conscious decisions on their behalf
X. NON-DISCLOSURE. Client acknowledges and confirms that this contract must remain
confidential. Except as may be legally required by competent authority or applicable statutes,
the parties hereto shall not make any unauthorized disclosures of the terms and conditions
embodied in this contract.
XI. ACCEPTANCE OF TERMS AND CONDITIONS OF EMPLOYMENT. Client’s affixing of his
signature on the herein Service contract means that: Client has read and fully understood the
terms and conditions hereof and accepts the same;The terms and conditions for the
regularization of his employment have been clearly communicated to and accepted by him/her
at the time of his engagement.
