What is it: A tool to capture data about Tablet, SIM Card and Power bank along with
their status.

What is being collected: No Each Material Received, Functionality status and

Availability.

Other details: The tool has sections to capture data about the Implementation and
Infrastructure Status that can generate reports like %of Functional Tablets, %HPs that
started service using the App and %HCs with proper feedback that can be used for
further intervention.

Who is collecting them: Woreda Health Office and Health Center

Use case - 3: Case detection and Treatment Completion Rate of Leprosy

Objective: To capture data regarding Leprosy cases and Its Treatment status.

What is it: A tool to capture data about Leprosy case activities, Contact Screening and
Treatment completion status along with Its Type, period, age and sex.

What is being collected: Type of Leprosy cases with age and sex disaggregation,
Contact Screening, Disability Grade and Treatment Completion rate.

Other details: The tool has sections to capture data about the number of peoples
affected with Leprosy with its type and Treatment completion rate. Also, used to capture
number of contact persons with the case. This can later be used to generate relevant
information like
 Leprosy Notification per 10,000
 Grade II Disability among new Leprosy Case
 Leprosy treatment rate
