What: Confiscated orange wire not surrendered

When: April 30, 2022

Who: FIS Crew T – 1501 and T - 1603

Where: MERALCO MANILA SECTOR

Why: Last Saturday, May 7, 2022. DUS FIS Project received a report from MERALCO and

ordered an immediate action and investigation towards the incident happened involving two
of FIS crew (team – 1501 and team 1603) for allegedly confiscation of orange wire but not
deliver and surrendered at MERALCO Manila Sector.
The following personnel that involved are as follows:

How: A complain report received by MERALCO Manila Sector. Involving FIS crew 1501 and crew
1603. The complaint is about the confiscated orange wire at Lopez Blvd. Quezon City.
As per investigation, after the assignment was done the crew’s return to Sector to surrender all
confiscated as accomplishment for that day. And upon inspection and documentation, no
orange wire were found on various kinds of wire surrendered. Thru verification to crew’s Daily
Accomplishment Report (DAR). Crew 1501 and crew 1603 did not surrendered the said orange
wire.
On follow up investigation, Project Supervisor went on site to gathered evidence. They got a
copy of CCTV footages from nearby gas station. And upon reviewing the footages, they were
seen with bundled of orange wire and was also seen loading to their service vehicle.
