WAIVER

I Jacquelene DC. Lim parents of Timothy John DC. Lim, allow our child to participate in the
INTERNSHIP/ON THE JOB TRAINING Programs of the College of Computer Studies of
Pambayang Dalubhasaan ng Marilao for 2 nd semester Academic Year 2022-2023.

I undertake not to hold the Pambayang Dalubhasaan ng Marilao or any of its officers or
administrators liable for any damage or injury which may be suffered by our child or caused
to third persons by any act attributable to the negligence or willful violation of rules by our
child or of any third person.

This waiver has been signed voluntarily and with full awareness of our rights under the law.
