INCIDENT REPORT

Type of Incident: SPOILED FRESH MILK

Inclusive Date & Time of incident: DECEMBER 29, 2021 @ 9:35 AM

NARRATIVE DETAILS OF INCIDENT:

At 9:35 A.M. of December 29, 2021, I, Mrs. Mylyn E. Sereno, the Feeding Coordinator of
Naval Central School SPED Center, received 301 pasteurized bottled milk from the supplier,
equivalent to five (5) days supply intended for the Naval Central School SPED Center
learners/beneficiaries.

Upon delivery of the said pasteurized bottled milk, I, together with the other school’s feeding
coordinators, witnessed that the bottled milk inside the buckets were frozen (with ice) to
prevent spoilage.

Yet, upon inspection, in preparation for the distribution, I found out that the milk appeared to
be yellowish in color, when I tasted it, it was sour and it has as foul (different) smell. Still, I
distributed the milk to the parents who were around and asked there observations on the taste
of the milk. They all told me that the milk was spoiled.

I immediately reported the above findings and observations to the District consignees and
District Nurses. I received instructions from them to hold the distribution of milk to our
learner/beneficiaries and wait for further instructions, while they coordinate with the supplier
and SDO staff coordinators.

Finally, the nurses instructed us not to distribute the spoiled milk for the safety of our
learners/beneficiaries. On the other hand, the supplier assured us of a 100% replacement of
the spoiled milk which was then delivered in good condition last February 10, 2022.
