For each practicum experience, please document the PCGs and/or CCPOs that were the focus of the experience (these are found in the course syllabus), and provide a detailed description of the experience that clearly illustrates each of the objectives. Objectives: CCPO #4 PGC #6 Direct patient-centered care through advocacy, interprofessional communication, collaboration, and delegation. Description: My preceptor and I had to assist a patient who was coded for cardiac arrest. When the nurse informed us that the patient become bradycardic and eventually went into pulseless electrical activity, we have to communicate effectively with other healthcare professionals who came to assist and collaborate with them efficiently. Since my preceptor was a supervisor, we also had to delegate some of the tasks to other nurses.

Objectives: CCPO #3 PGC #4 Integrate information management technology to improve patient outcomes. Description: With my preceptor, we had to audit the medical record system of the facility. I was able to learn how to audit the carting, patient care documentation, medication errors, and sentinel events that happened in the facility. We were able to find some discrepancies with the medication delivery system and medication errors that happened that same week.

Objectives: CCPO #5 PGC #8 Practice professional nursing within an ethical framework. Description: On that day, I was answering some of the phone calls at the nursing station. My preceptor had to attend a meeting and I was learning from the other nurses until she comes back. A person called asking about a patient and said he is the brother of that patient. He asked f I can provide him with updates on the status of the patient. Because of my professional nursing practice and ethical knowledge, I knew it was against the laws of HIPPA to provide information without screening and identifying the caller. When I ask the called for a medical record number, he didn’t have one and I politely told him that I can’t provide him with that information. As it turned out, the patient didn’t have a brother and didn’t want his information to be shared with anyone.

Objectives: CCPO #4 PGC #7 Integrate health promotion and disease prevention practices to positively impact the delivery of healthcare to diverse populations. Description: Following my preceptor, I was able to provide health promotion education to a diverse population, mainly from low socioeconomic societies. I was able to provide disease prevention education to several Hispanic and African American populations regarding diet, exercise, hypertension, diabetes, and stroke prevention. I educated some elderly patients regarding prostate cancer screening and smoking cessation.
