AFFIDAVIT/DECLARATION

Known all the mean by these present that through this affidavit we, the company named
Hospital Management Company. 4th Floor, Bahria canal heights, Bahria Town, Lahore, Pakistan
does here by declare that:
Company has never been debarred, black listed or suspended in the past nor it expect to be
debarred, black listed or suspended against its ongoing projects through any judicial or other
proceedings and also not blacklisted by any of the provincial or Federal Government
Department anywhere in Pakistan.
The company will full fill Delivery period as mentioned in the tender document.
All Software we will supply if tender a lot to us will compliance to the technical specification of
tender document.
The company on submitting this affidavit is legally bound to pay penalties as stated in the
tender if at any stage it is found that it has been debarred, black listed or suspended through
any judicial or other proceedings.
