⮚ Cardiovascular disease (CVD) has a greater impact on Indians
in their most productive midlife years, occurring at least a
decade earlier compared to Western populations.

PROBLEM STATEMENT:
⮚ Elderly and discharged patients often struggle to visit
hospitals for regular check-ups, which can result in delayed
diagnosis and treatment.
⮚ To address this issue, a low-power, portable, and accurate
post-hospital monitoring system is required.
⮚ Such a system can improve access to care, reduce the
burden of hospital visits, and provide timely medical
attention.
⮚ Additionally, the system can allow patients to access their
medical reports at any time and any place.

Objectives:
➢ To Improve patient’s outcomes: It detects potential health problems
early and provide timely interventions to prevent complications.
➢ To Increase patient’s engagement: Patients may feel more
empowered and engaged in their own care, as they have access to
real-time data about their health status.
➢ To Reduce healthcare costs: It reduce the need for in-person
healthcare visits, which can be costly and time-consuming for
patients.
➢ To Enhance communication between patients and healthcare
providers: It can facilitate communication between patients and
healthcare providers, allowing for more frequent check-ins and
better coordination of care.

EXPECTED OUTCOME
➢ Remote health monitoring with blockchain technology improves
patient outcomes by detecting potential health problems early and
providing timely interventions to prevent complications.
➢ Patients who use remote health monitoring systems with
blockchain technology feel more empowered and engaged in their
own care, as they have access to real-time data about their health
status.
➢ Remote health monitoring with blockchain technology reduces
healthcare costs and enhances communication between patients
and healthcare providers by securely storing and sharing patient
data among authorized parties, reducing the risk of data breaches
and fraudulent activity.
