1. GROUNDING / STRANDING

In the event of grounding or stranding of the vessel, following action is to be taken:


Stop Engines.
Sound General Emergency Alarm.

Muster all crew and Check for any casualty.

Consider use of Anchor.
Close all watertight compartments & Fire doors.
Exhibit lights/shapes / sound signals and update AIS Status.

Switch On all Deck lights at Night
Ascertain the exact geographical location of the vessel and note down date/time of
occurrence/Vessel’s Speed / Engine Rpm/ Depth Sounding just prior the Incident.

If vessel is under pilotage, obtain his written statement about the incident, along with the
statement of facts from the Bridge / Engine Teams and Other Crew Involved.
Note down wind, weather, sea and current conditions.
Ascertain state of tide at grounding and tidal conditions i.e. time and range of next high

water and low water.
Sound bilges and Tanks, check hull for damages
Take soundings around vessel at 6 different points, in reference to the centreline of the
vessel and assess grounding point/area.

Ascertain Fwd, Aft & Mid ship draft before and after grounding.
Depth of water at location of grounding considering charted depth and height of tide.
Inform Head office/DPA, vessel’s agent, owners, charterers and other parties using the
ef

Emergency Response Procedure.


Check if vessel has developed any list. If yes note down how much and whether it is
increasing.

Check if any local assistance, like tugs are available and advise agents to make
arrangements for keeping same on stand-by.
Check for possibilities of Environmental Pollution or damage to cargo due to water
ingress.

Broadcast Distress Alert and message if the ship is in grave and imminent danger and

immediate assistance is required, otherwise broadcast an Urgency message to ships in


vicinity
Switch to high cooling water suctions.
Check proximity to shore and inform the nearest shore station / MRCC.
Ascertain nature of sea bed and shore line.
Check if any change in sounding of double bottom tanks, hold bilges and voids.
Check if any apparent damage to hull structure, in case of flooding decide on what is the
best possible action.

Consider reducing ships draft/trim by de-ballast or Transferring ballast and/or transferring

2. FLOODING

In the event of flooding, following action is to be taken:


Raise General Emergency alarm.
Call the Master and inform bridge

Muster Crew, Take head count and check for any casualty.

Close all watertight compartments & Fire doors.
Ascertain the exact geographical location of the vessel and note down date/time of
occurrence just prior the Incident.

Ascertain location and extent of the flooding and is the pumping arrangement for the
compartment able to cope with the ingress of water.
Determine the probable cause of the flooding.

Check resultant list/change of trim and if adjacent compartments affected by the leakage.
Check the permeability of the compartment that is flooded.
Verify disposition of bunkers and ballast, cargo type, quantity and distribution.
Carry out damaged stability calculations taking into account the flooded compartment,

and the action required (ballasting / de ballasting) to counter adverse trim / list.
Ascertain wind, weather, sea and current conditions and forecast.
Evaluate whether safety of the vessel is endangered and evacuation of personnel
necessary.

Check proximity of shore and nearest port of refuge as well as the possibility of beaching.
Verify vessel’s equipment and machinery are operational.
Check whether there is any threat of oil pollution.

Check if other ships are in the vicinity.


Prepare lifeboats for launching.
Notify the nearest coast station of the incident.

Explore the possibility of effecting temporary repairs to control the situation.

3. COLLISION

In the event of a collision / contact with another vessel or a fixed or floating object, following
action is to be taken:
Raise General Emergency alarm.

Check if any tanks, holds or compartments are penetrated above or below the waterline.

Check if there is any oil spill or threat of oil spill.


Alert nearest shore stations/MRCC.

Ascertain if the vessel is in any imminent danger and whether the stability and
seaworthiness is affected.

Check whether any fire has resulted owing to the collision. If yes in which part of the
vessel and what preventive measures are required.
Determine if the vessel’s maneuverability been affected and are all engine, deck and
navigational equipment/machinery operational. If required, check for the possibility to
anchor the Vessel.
Ascertain if there is any need of evacuating the vessel’s personnel.
Ascertain if any assistance, like salvage, is required.
In case collision has occurred in poor visibility conditions or in high density traffic areas

has the appropriate coastal station been notified especially in areas where vessel
reporting systems are operational e.g. English Channel etc.
Check and note down the disposition of ballast, fresh water, cargo and bunkers on board.
Obtain weather forecasts / prospects.
Try and establish communication with the other vessel.

Issue a letter of protest to the other vessel, holding them responsible for all damages and

consequences.
If a similar letter has been received from other vessel it must be endorsed as “signed as an
acknowledgment of receipt only, and not as an admission of liability”.

Try to determine extent of damage to the other vessel / fixed or floating object.
To render to the other vessel, its master, crew and passengers (if any) such assistance as
may be necessary to save them from any danger caused by collision; and to stay by the
other ship until he has ascertained that it has no need of further assistance.

If pilot is on board, obtain his full name and written statement.
Establish cause of collision / contact and write down names of all staff on watch at that
time on the bridge and in the engine room. Obtain their written statements.
Master’s statement has to be compiled and relevant entries made in the official log book.

Keep Life Boats / Rescue Boats in readiness for launching if necessary

4. ABANDON SHIP

In the event of having to abandon the vessel, following action is to be taken:


Sound General Emergency Alarm.
Muster all crew, Take Head Count, ensure all personnel on board are present.

Inform Engine Room and start emergency generator.

Engine room party to shut off Main Engine, all auxiliary machinery and pumps, all over
side discharges.
Shut off all water tight doors and ventilators.

Abandon ship orders must be given verbally by the Master.
Ensure distress message has been sent out. Time permitting notify head office and
nearest MRCC.
Check if there are any other ships in the vicinity. If so attempt to establish contact.

Check proximity of shore and nearest port.

Ascertain wind, weather, sea and current conditions and forecast.

Prepare Life boats for launching and lower until the embarkation deck for crew boarding.

Preserve VDR or S-VDR records if not automatically protected
If Possible, Take along with you current log books, ship’s certificates and crew documents,
at the time of abandoning the vessel.
Place additional water and blankets into the lifeboats.

Activate the EPIRB and place the SARTS and GMDSS walkie talkies in the boats.
Ensure no crew members are left onboard.
Launch the inflatable life rafts as well as the lifeboats, Only on Master’s Orders.

Get clear of the vessel and gather all survival craft together. Follow the guidelines of the
survival manual.
Institute survival techniques and keep re-sending distress calls.

5. MAN OVERBOARD

In the event of a man overboard, following action is to be taken:


Release the MOB Marker on the side where the person has fallen overboard.
Engage in hand steering with two steering pumps, give helm hard over to the side man

speed and direction and time. On vessels equipped with ECDIS activate MOB mode.

on Ship’s alarm System.

Muster Crew and confirm Man overboard.
Post extra look outs with Binoculars and instructions to maintain a continuous watch on
the man overboard.

Sound three prolonged blasts on the ship’s whistle, repeat as necessary and hoist Signal

Inform engine room, get engines ready for Manoeuvring.
Broadcast URGENCY message to all vessels in vicinity, and share authorities if search is

Ascertain wind, weather, sea and current conditions and forecast as well as air and
seawater temperatures.
Commence a recovery Manoeuvre such as Williamsons turn.

Modify AIS status to ‘restricted manoeuvrability’.


Muster rescue boat crew and prepare the rescue boat for launching.
Rig pilot ladders/nets to assist MOB recovery, If required

Once Casualty recovered from water then Render First Aid, Treat for Shock/Injury. In case
of cold water immersion, treat for Hypothermia.
Preserve VDR or S-VDR records if not automatically protected.

6. FIRE IN ENGINE ROOM

In the event of a fire in the engine room, following action is to be taken:


Sound general Emergency Alarm
Inform bridge / Master

Muster all crew, Take Head Count and check for missing or injured personnel and
evacuate all injured persons to safe location on deck.

Put Engines on standby or stop, and steer vessel away from the Traffic areas or congested
waters.

Ascertain the exact geographical location of the vessel and note down date/time of
occurrence of the Incident.
Adjust ships course and speed to minimize wind effect.
Determine location and extent of fire/explosion.

Switch off Engine Room blowers and shut all ventilation and openings to the engine room
including funnel flaps in case of Engine Room Fire.
Check Fire pumps are running and keep emergency fire pump ready.
Start additional generator for power, if necessary.

Initiate firefighting measures using fire hose and extinguishers as appropriate.
Try to establish probable cause of the fire and/or explosion.
If vessel in port, inform agents and seek assistance of local fire brigade.

Ascertain if in case of fire only is there any risk of explosion.


Prepare to release CO2 or use other fixed fire fighting medium provided.
Prepare to shut off quick closing valves. Ascertain whether vessel’s equipment and

machinery are operational.


If fire already extinguished, ascertain extent of damages to the vessel and/or equipment.
Note down date/time fire extinguished.

If fire has not been extinguished ascertain whether it is restricted or spreading.


Shut off quick closing valves for all tanks in the engine room and release CO2 or other
fixed firefighting medium after taking a New Head count.

Commence boundary cooling.

Ascertain whether any threat of oil pollution.


Ascertain if safety of the vessel is endangered and evacuation of personnel necessary.
Disposition of bunkers and cargo and whether cargo is affected and if dangerous goods on
board.
If other ships are in the vicinity issue relevant warnings to them requesting for assistance.
Notify the nearest coastal station of incident.
Check proximity to shore, possibility of dropping anchor and nearest port of refuge.
Notify Head office, Vessel’s agents, owners, charterers and other parties as appropriate.

7. ACCOMMODATION FIRE

In the event of a fire in the accommodation or other space other than cargo hold or
machinery space, following action is to be taken:
The first person who notices fire within the Accommodation spaces must immediately

sound the Fire Alarm using nearest Fire Alarm switch.
Inform Bridge, Master and Raise General emergency alarm.

Muster all crew, take head count, check for missing/injured crew members
Close all Accommodation ventilations, fans, blowers, entrances and fire doors to isolate
the area from supply of air.

Shut off electrical supply to area of fire
Assess fire and determine:
- The class of Fire.

- Appropriate extinguishing agent
- Appropriate method of attack
- How to prevent the spread of the fire
- Fire-fighting methods to adopt and assignment of necessary personnel for same
Initiate firefighting measures using fire hose and extinguishers as appropriate.

Start Boundary cooling from available 6 sides near to the seat of the fire.
Ascertain the exact geographical location of the vessel and note down date/time of
occurrence of the Incident.

8. FIRE IN DECK HOLD / CONTAINERS (NON TANK)

In the event of a fire in the cargo holds or containers, following action is to be taken:
The first person who notices fire in any cargo hold must immediately sound the alarm
using nearest Fire Alarm switch or contacting bridge or the Duty Officer.

Inform Master and Engine room.
Muster all crew, take head count, check for missing/injured crew members.

Close down ventilations fans, all booby hatch entrance and hatch covers of the cargo hold.
Start fire pumps.

Start Boundary cooling.
Monitor the Hold temperature and adjacent compartments temperature.
Assess fire and determine depending on type and nature of cargo carried with reference
to Cargo Safety data Sheets

Remove flammable material from neighbouring compartments/spaces, If possible.
Ascertain the exact geographical location of the vessel and note down date/time of

If fire is in a container determine the contents of the container from the manifest and

explore the possibility of jettisoning the container.


To check if fire is already extinguished, checking of temperatures is essential. Do ensure
proper records of temperatures are maintained. Check temperatures at decks, bulkheads,

in the air space of cargo hold and inside sounding pipes, with boundary cooling stopped.
The protection necessary for re-entry after a fire is typically BA set and protective

clothing, similar to that used for carriage of dangerous goods. Lights used should be
strictly intrinsically safe. Ensure fixed fire fighting system release system is checked to
prevent accidental release prior to / when men are making man entry into the cargo hold.
If fire already extinguished, ascertain extent of damages to the vessel and/or equipment.
Note down date/time fire extinguished.
If fire is spreading, evacuate all personnel from the area, take head count, shut all
ventilation and openings to the space.
Operate the fixed firefighting system, after the head count.

Once fixed firefighting system has been used in a cargo hold, it must NOT be opened /
ventilated at sea. The hold / vents must only be opened once vessel is alongside berth,
with local shore fire service in attendance.
Notify the nearest coastal station of incident.
Check proximity to shore, possibility of dropping anchor and nearest port of refuge.

9. FIRE IN PUMP ROOM (OIL / CHEM TANKER)

In the event of a fire in the pump room, following action is to be taken:


The first person who notices fire within the Engine room area must immediately sound
the Fire Alarm using nearest Fire Alarm switch or contacting bridge

Leaks from the cargo pipelines result in clouds of gases. With time this cloud grows and when
it comes in contact with any source of ignition, catches fire and travels back to the leaking
point. This leaking point then continues to burn with more evolution of gas in form of a jet

of crew in attendance and move vessel to a safe location. Where times permits,
attach anchor buoy & recovery line prior releasing from bitter end.

35. RESCUE OF PERSONS TRAPPED IN ELEVATOR

In the event of Person getting trapped in a ship’s elevator, following action is to be taken:
Establish clear lines of communication between the person inside the elevator cabin
and the person outside.

36. HELICOPTER OPERATIONS


In the event of Helicopter operations, following action is to be taken:
Consider the Position, speed, time, wind direction and velocity, Contact the
Helicopter, and decide on the rendezvous point.

If beyond Helicopter range divert vessel to rendezvous point.
Inform Engine room, Keep engine ready for Manoeuvring.

Instruct stand-by crew not to touch any helicopter equipment or descending person

static electricity. Normally the helicopter will “Land” its hoisting wire onto the deck
to discharge any static electricity.
Instruct stand by crew that they must only approach a helicopter from the front – in

view of the pilot.


Instruct stand-by crew not to make fast the helicopters hoisting wire and to make
sure it does not get entangled.

Adjust Course and speed to keep the funnel smoke away from Helicopter approach
and Landing area.

37. FAILURE OF CARGO CONTAINMENT SYSTEMS ON TANKERS

In the event of Cargo Containment system failure, following action is to be taken:


Raise Alarm.
Transfer contents of affected tank to another tank with ullage and preferably with

compatible cargo. Else consider Jettison of cargo in accordance with the procedure.
Temperature sensors in the cargo tank hold space will activate an alarm at the

prescribed temperature if the steels in this space becomes cooled below their
service temperature. Hold bilge alarms & gas detectors will activate.
Use cargo eductor to pump out the hold bilge into the other tank.

Monitor Hold pressure and Cargo tank levels.
Purge the cargo hold with inert gas in accordance with the procedure Cargo Hold
Inerting.

38. RESTORING STABILITY
