Types of Incorrect Prescriptions


Objectives:
1. To be aware of the different types of incorrect prescriptions
2. In the context of the laws such as RA 10918, student-pharmacists should be able to identify if the
prescriptions are valid.
3. Determine the appropriate course action plan if there are violations.

Pharmacists and owners of drugstores, pharmacies and
other drug-dispensing establishments are only allowed
to dispense drugs if there is a written prescription

When a prescription is not signed in ink by the
appropriate practitioner giving it.

1. Research on the types of Incorrect Prescriptions.
2. Create a list of criteria to identify which incorrect prescription it belongs to.
3. Provide a check mark if it is appropriate.
4. Each member should provide 1 Prescription, label the different parts of the prescription, and identify if it’s a
valid prescription or an incorrect prescription.
5. For each prescription, please indicate if it's valid to dispense or not with rationale.

The prescription can be dispensed, because multiple prescribed drugs in one prescription today is
considered valid especially to the elderlies who are taking maintenance or a lot of medication and the
drug prescribed is valid despite only the generic name is written/type which for the purpose to
promote the use of generic brand which has lower cost and the number of medications is one of the
reasons why only the generic is being prescribed.

The Prescription will be dispensed. The Prescription paper consists of the requirements that are
needed which are the Prescriber’s Information, Date it was written, Patient information,
superscription, inscription, subscription, and the Signatura which makes it dispensable and valid. The
generic name is only being indicated which means no brand name was included. No brand name
inside of a prescription paper can still be dispensed as Generic drugs and Branded drugs have similar
drug compounds. Generic Drugs are convenient for the patient or the customer as it is much cheaper
and reliable. The prescription paper above indicates 3 drugs to be taken for the patient instructed by
the Physician. This is considered an Erroneous prescription, but still valid as Cetirizine Syrup,
Ambroxol Syrup, and Co-Amoxiclav which is an antiallergic, that relieves cough, treats runny nose,
watery eyes, and sneezing.

-Medications are compatible with current medical conditions, with other medications being taken,
contraindication, allergies
-Generic name precedes brand name
-Brand name is in parenthesis
-Generic name is written
-Both generic name and brand name are legible
-Generic name corresponds to the brand name
-One with only the generic name is written is legible

Dispense, the prescription since it meets the legal requirement. Although certain medications do not
contain a brand name but can still be dispensed.

- Valid prescription

b. Dispense or not? Explain.


- Dispense. due to the fact that the prescription is exact in every way. The doctor further advised against
drinking alcoholic drinks while taking metronidazole (flagyl), since it may result in nausea, vomiting,
flashes, and perhaps chest pain. The printed generic and brand names are identical. Also, the dosage
of the aforementioned medication is ideal for the patient's prescription treatment.

- Valid. The medication is valacyclovir, a medication for antiviral drugs used to treat viral infections.
This medication is for herpes.

b. Dispense or not? Explain.


- Dispense. The prescription is all correct. It meets the legal requirement for a prescription. The
prescription only writes the generic name which is also correct because the patient has the right to
choose between a generic ( a much cheaper price ) compared to brand name which is a bit expensive.
Because Valacyclovir is available both as a brand and generic name.

Dispense. The prescription clearly stated the medications and information about the patient as well as
the prescriber’s. All the parts of the prescriptions are followed

- The prescription is valid, because it has all the correct details.

b. Dispense or not explain?


- It should be dispensed, because the tablet in their has a medication of an antibiotic which is for
bacterial infections and it has the proper days of how many times should be taken, because an
antibiotic drug should be taken 2x a day and should be taken in 7 days and some are 14 days in order
to ensure that all of the illness that cause by bacteria are killed or prevented from multiplying. For that
vitamin c + zinc it is for the immune system. It has also the signature of the doctor which has the
authority to be dispensed.

- The prescription is valid, because it has all the correct details.
      

b. Dispense or not explain?


- Dispense. In addition to information on the patient and the prescriber, the prescription was quite clear
regarding the meds. Every instruction is followed in its entirety.

- Dispense. The drug and patient information were clearly indicated in the prescription. Although
certain brand names are missed, it is convenient for the patient since the pharmacist may give a
considerably cheaper and reliable alternative.

- It should be not dispensed. Although the information of the patient is stated clearly and the medicines
prescribed are clear and correct in terms of their dosage, the details of the prescriber written is
incomplete. No signature and no license number.
