NOTE: Every RMA/DOA No. is valid for two weeks from the date of issuing
1. Please put the RMA/DOA form that contains RMA/DOA number inside the box, and also
paste the RMA /DOA Carton Label (sent by QNAP with RMA/DOA number) on the external
carton box.
2. List the value of the product on the invoice as zero (or a very low value,
say $5). Otherwise, additional charges will be levied by customs (which will be borne by
the sender). We suggest that you write “Goods with no commercial value, returned for
repair” on the shipment invoice.

Extremely Important:
1. RMA/DOA No. will not be issued if this request form is incomplete.
2. Don’t return the complete unit back to us if it is only a problem on specified part. For example, if the
problem is on the motherboard, just return the motherboard back to us. And if the problem is on
the power adapter, just return it.
3. Please complete this form (except RMA/DOA No.), and send back by email to QNAP Technical Support
in order to receive an RMA/DOA number.
