Re: Declaration Letter on your Cuckoo Product King Top (“Product”)

With reference made to the above matter.

We wish to inform you that the specifications of the Product you have installed in your premises
is to cater the normal household usage. For normal corporate client, we would recommend other
range of products that are more appropriate to serve your better on the usage. That to say, the
Product shall be undergone total of three (3) services in a year. In the event any additional
service(s) that you have requested to be conducted, a service charge will be incurred for you to
make payable to Cuckoo International (MAL) Sdn Bhd.

By you agreeing to subscribe and install the said Product, you fully understand that specifications
of the Product are designed for homes, the tank capacity for cold water is 1.0 litres, room
temperature water is 0.3 litres and hot water is 1.0 litres. The tank capacity may not be able to
accommodate the high dispensing amount of water at short span of time.

You hereby unconditionally and irrevocably undertake to indemnify Cuckoo International (MAL)
Sdn Bhd against all losses, liabilities, damages, costs and expenses whatsoever arising out of
Product, save for the warranty period of the Product.

Kindly indicate your acceptance by signing on the acknowledgement on this letter.
