I, PSSG. MALIAWAO D. MINDALANO Filipino, of legal age, member of the Philippine
National Police and presently assigned at Madamba, Lanao del Sur, Madamba Police
Station, do hereby depose and say, that:
1. On November 5, 2022, I filled out and submitted the inventory of Uniforms
in compliance with the directive for the record of PNP uniforms in
possession of each PNP member;

2. I have complied with the foregoing directive by submitting the list, kind and
quantity of PNP uniforms in my current possession;

3. I attest that the submission I made were truthful and honest to the best of my
knowledge; and

4. I am executing this Affidavit to attest to the veracity of the foregoing

statement for whatever legal purpose it may serve.
