HOME – CENTER AGREEMENT

 Attendance is a must. Try your best to be on time. Attendance will be check daily.

 Inform the ECCD Provider if your child will be late or if she/he is not feeling well.

 Wear your i. d. always with in the Day Care Center premises.

 Parents must attend monthly meeting every second Monday of the month.

 Participate in every activities of the day care center/municipal/district/provincial.

 Observe cooperation from among the parents.

 Be friendly always.

 Parents are not allowed inside the room wHen classes are going on.

 When signal no.1 is raised by the PAGASA classes automatically suspended.

 Registration fee PHP 150 upon enrollment.

 User’s fee monthly will be PHP 150.00

 Contribution allowed upon the request of the parents as needed for the activities that need to be
funded.

PARENTS AGREEMENT
We the parent’s committee and members of Brgy. ______________ agree that will provide support
for Child Development Worker (CDW) to pay the User’s Fee of monthly of our children in the amount of
PHP 150.00 that will pay the amount at the 2nd week or at the end of the month in order to reach
the satisfactory rating of the honorarium receive by our CDW. And a Registration Fee of PHP 150.00
upon enrollment.
In consist are attached our signature for the approval and implementation.
