AFFIDAVIT OF UNDERTAKING

I, Alfredo R. Espinosa, of legal age, Filipino and duly authorized representative of Ayala-
Laguna Transport Cooperative, after having been sworn in accordance with Law, hereby declare
that:

1. That Ayala-Laguna Transport Cooperative is filing an Application for Consolidation

of Certificates of Public Convenience pursuant to Memorandum Circular No. 2022-
059;

2. That the cooperative undertakes to substitute the existing authorized units within the
period;

⁎ 3 months – 25% of the total number of authorized units

⁎ 6 months – 50% of the total number of authorized units
⁎ 9 months – 100% of the total number of authorized units
Reckoned from the date of receipt of the Application for Consolidation

3. That Applicant further undertakes that the substitute units shall be compliant with the
specifications and standards under OFG and DTI-PNS;

4. I execute the affidavit to confirm the truth of the foregoing.
