CONTRACT OF VEHICLE

LOAN AND FINANCING

KNOW ALL MEN BY THESE PRESENTS:

This contract is entered into by and between:

SURECHOICE PHARMA PHILS, INC., a corporation duly organized

and existing under the Philippine laws with office address at 2 nd Floor
Eurocare Pharma Office, Km. 21 East Service Road, Cupang, Muntinlupa
City, represented herein by its General Manager, MR. RODERICK N.
VILLANUEVA, hereinafter referred to as the “PROVIDER”;

- and -

___________________________, of legal age, Filipino, with postal address

at ______________________________, hereinafter referred to as the
“BENEFICIARY”.

WITNESSETH:

WHEREAS, the Beneficiary is qualified to avail of the Car Loan and

Financing Program of the Provider;

WHEREAS, the Beneficiary has signified his/ her acceptance of the same;

WHEREFORE, the parties hereto have agreed to the following terms and
conditions:

1. The Beneficiary may choose between Hyundai Accent OR Hyundai I10;

2. The downpayment (which amount will be included in the full amount of the
vehicle) will be paid by the Provider;
3. The Provider and the Beneficiary will share in 60/ 40 proportion for the
payment of the monthly amortization;
4. The Beneficiary’s share in the monthly amortization will be deducted from
his monthly quota. If and when the monthly quota is not met, this contract
will be extended month on month until the Beneficiary reaches his quota.
5. The Beneficiary shall not be deducted with more than two (2) shares for the
monthly amortization, unless it is authorized by him in writing;
6. The Beneficiary binds himself to be with the Provider for five (5) years, unless
sooner terminated by the Provider for just and lawful cause/s or in case of
resignation or separation from employment, in which case, the vehicle and
the payments made by the Beneficiary will be forfeited in favor of the
Provider;
7. Maintenance of the vehicle will be for the account of the Provider during the
five(5) year lock-in period; and
8. The validity, interpretation, construction and performance of this Contract
shall be governed by the Laws of the Philippines.
 I have read, understood and accept in full the aforementioned terms and
conditions of this Contract, as set out in this document.

This instrument refers to a CONTRACT OF VEHICLE LOAN/ FINANCING

which consists of three (3) pages, including the page whereon the acknowledgment is
written, and which is signed by the parties and their instrumental witnesses on each
and every page thereof.
