Guarantee Letter(GL) Requests


via My AIA App

Guarantee Letter Requests via My AIA App (Step 0)

STEP 0

All GL request starts with Step 0 (Let’s Begin) :

1. Appointment Date – GL Type and eligibility is


calculated after the date is selected.

2. Request For – If the GL is for Member or


Dependent.

3. GL Type – Refer to the dropdown selection. Only


the valid product type and eligible GL for that
member / dependent is displayed here.

4. Company Name – Select the member’s company


name before proceeding with the request.

STEP 0
In total, there are 7 types of GL in the “Select GL
Type” dropdown. But it will only be displayed if
member has that benefit / eligibility to request for
that type of GL.

When will GL Type be displayed:


• Specialist Care – Auto display if u have this
benefit
• Post Hospitalization – with GHS benefit and a
valid FGL within the post follow up period.
• Collection of Medication – with GSP benefit and
a Specialist NEW VISIT previously.

• Special Treatment (Dialysis, Physiotherapy &


Radiotheraphy) - First Time request for any 1
Procedural has to be requested by hospital via

Hospital Portal. For subsequent
treatment/procedure, member can request via 3
MyAIA under “Special Treatment”.

• Health Screening - Auto display if u have this 5
benefit
• Maternity/Pre & Post Natal - Auto display if u 6
have this benefit. 7
• Government Hospital - Auto display if u have this
benefit.

1 Specialist Care
STEP 1
Specialist Care GL:
New Request –
Based on the GL type
Without any previous history. Data
selected, Step 1 is different input required for the GL request.

Specialist Care GL:


Follow Up –
With previous outpatient “New
Visit” history.
Select the initial SPGL number for
the follow-up GL request. No other
data input required.

2 Post Hospitalisation 3 Collection of Medication
STEP 1

Based on the GL type


selected, Step 1 is different Post Hospitalisation GL –
for each GL selected. Select the FGL number for the Post
Hospitalisation GL request. No other
data input required.

Select the initial SPGL number for
the GL request. No other data input
required.

4 Special Treatment

Based on the GL type Special Treatment GL –

Require to select Provider Name
and Doctor’s Name

Upload the required Review and confirm the GL request. Submission Confirmation
