CONTRACT OF TRANSPORT SERVICE

KNOW ALL MEN BY THESE PRESENTS:

This CONTRACT OF TRANSPORT SERVICE is made and executed by and between:

UNIVERSITY OF, a public, research, coeducational, regional state

university, with address at _______________, represented herein by its college
faculty, ______________________, of legal age, Filipino and resident of
_________________, hereinafter referred to as the “FIRST PARTY”;

______________________, of legal age, Filipino, and a resident of

__________________________, duly represented herein by its/her General
Manager ______________________, of legal age, Filipino and resident of
____________________ hereinafter referred to as the “SECOND PARTY.”

WITNESSETH; That

WHEREAS, the FIRST PARTY is desirous in engaging the services of the SECOND
PARTY in transporting second year engineering students to ______________________ for their
Field Work;

NOW THEREFORE, for and in consideration of the foregoing premises, the Parties
hereto have agreed as they do hereby agree, as follows:

1. That the SECOND PARTY will provide (1) bus transport service to the second year
engineering students of __________ from ______________ to
______________________, and vice versa (one roundtrip only);

2. That the FIRST PARTY shall pay the SECOND PARTY an amount of
