Instruction

TEPID SPONGE BATH

MATERIALS/ EQUIPMENT:

1. Basin with water


2. Wash cloth
3. Bath towel
4. Thermometer
5. Gown/ camisa/ pants/ and linen required
6. Screen Panel

PRECAUTIONARY MEASURE/S:

1. Perform hand hygiene.


2. Wear Personal Protective Equipment (PPE).

STEP-BY-STEP PROCESS RESPONSIBLE PERSON/S


1. Take the temperature of the patient prior to procedure Nurse, Nursing Attendant
2. Explain the procedure. Nurse, Nursing Attendant
3. Prepare materials needed. Provide privacy. Nurse, Nursing Attendant
4. Loosen client’s gown/camisa. Nurse, Nursing Attendant
5. Perform sponge bath several times by:
a. Exposing one part of the body one at a time, gently sponge with
tepid water for approximately 5 minutes. Begin with face and
extremities then back. Nurse, Nursing Attendant
b. While sponging place cool compress on forehead, axilla’s and
groin.
c. Pat each area to dry after sponging and avoid rubbing with the
towel.
6. Monitor temperate every 10 minutes while sponging. If it is normal
discontinue the procedure Nurse, Nursing Attendant
7. Remove soiled client’s gown and replace with new one Nurse, Nursing Attendant
8. Remove the top sheet, replace with a new one, then tighten under the
bedding. Nurse, Nursing Attendant
9. Place the client in a comfortable position. Nurse, Nursing Attendant
10. Take the vital signs. Nurse, Nursing Attendant
11. Clean and store all the equipment used. Leave the area in order. Nurse, Nursing Attendant
12. Document. Nurse
