AUDIT CHECKLIST

1) License to operate / Business Permit


2) Fire Safety Inspection Certificate
3) BIR related reports / certifications
4) PhilHealth Certificate of Good Payment Standing (January - December 2022)
5) SSS Certificate of Good Payment Standing (January - December 2022)
6) Professional license (accreditation or driver's license) of your active and deployed
personnel
7) Accreditation with various organizations as applicable to the nature of business
(e.g. PADPAO, PCAB, DENR accreditation/certification, etc.)
8) DOLE reports for Year 2022:
- DOLE Annual
- Contractors/Subcontractors Report
- DOLE Annual MEdical Report (AMR) 2022
- DOLE Annual Work Accident/Illness Exposure Data Report (AEDR) 2022
- DOLE RKS Form/Establishment Report (reporting of separated and terminated
employees and/or application for flexible work arrangements)
- Completion of written OSH Program related to COVID-19 safety protocols
- Quarterly Family Welfare Program Report (for 200 employees
or unionized company)
- 13th month pay reporting (PD no. 851) to be submitted not later
than January 15, 2023
- Payout of deferred holiday payments not later than December 31, 2022
