VII. PROCEDURE FOR NOMINATION:

1. District Offices are required to submit to the Division PRAISE Committee


through the PRAISE Secretariat the pertinent documents of their
nominees for Stage 1: Paper Screening which will be subject to initial
evaluation by the Division PRAISE Committee.
Each district is allowed to submit one (1) nominee (who passed
though District Award Selection Committee and who ranked first - for
the following categories:
● Teaching Personnel (TI-III, MTI-II)
● School Heads
● Non-Teaching Personnel (SG 1-19)

SDO nominees covering Non-Teaching Personnel & Supervisors


shall be endorsed by the concerned Division / Unit Head.
2. The nominator shall use the official nomination form.
3. All nominations shall be sealed upon submission to the Division Office
Records Unit on or before the specified date addressed to the
Chairperson of the Division PRAISE Committee.
4. The District Award Selection Committee shall check the completeness of
all documents submitted before endorsement to the Division PRAISE
Committee for paper screening, validation, and shortlisting.
5. The District Award Selection Committee members shall screen, validate,
and deliberate the nominations using the criteria for paper screening as
basis for shortlisting the nominees per category.
6. The District Award Selection Committee members shall endorse the
shortlisted nominees (the top nominee – 1 per District) per category to
the Division PRAISE Committee through the PRAISE Secretariat.
7. The Division PRAISE Committee shall, likewise, screen, validate and
deliberate the nominees’ qualifications.
8. The Division PRAISE Committee shall interview the shortlisted nominees
per category and shall select the winners after final deliberation.
9. In case of a tie, the Chairperson of the Division PRAISE Committee
shall make the decision.
10. The Chairperson of the Division PRAISE Committee shall forward the
result to the Schools Division Superintendent for approval.

VIII. PROCEDURE FOR SCREENING AND EVALUATION:

1. Stage 1: Paper Screening

The Division PRAISE Committee shall conduct the paper screening using the
submitted means of verification (MOVs) on each specific criterion. Further,
nominees must submit the following documents:

 Duly accomplished nomination form signed by the nominator (see


attachments)
 Executive Summary (see attachments)
 Updated Personal Data Sheet (PDS)
 Attendance record as certified by the School Principal/ Division / Unit
Head / Human Resource Management Officer (HRMO)
 Updated Service Record duly certified by the School Head / Human
Resource Management Officer (HRMO)
 Submit a certification of the last three (3) performance rating periods prior
to nomination signed by the School Principal / Division / Unit Head /
Human Resource Management Officer (HRMO)
 Certification that the nominee has not been suspended for any
administrative, criminal, or civil offense/case signed by the nominee and by
the School Head / highest-ranking Administrative Officer or Legal Officer .
 Certification of No Unliquidated Cash Advances signed by the Finance
Officer/Accountant of the School/Schools Division Office.
 Milestone Accomplishment (max. of 2 mins. Audio-visual)
NOTE: Submit using A4 size Green folder for Teaching Personnel, Yellow
folder for School Head, Blue folder for Non-Teaching Personnel and Orange
folder for Supervisor nominees.

Kindly put Name, Position, School and District on the front cover. Also,
secure tabs on folder for easy reference and follow the recommended paper
organization below.
