A. The Role of Midwives in the Health Care System


Role is a set of levels that are expected to be possessed by people who are domiciled in
society (Anton, MM, 1988:667).
1. Definition of Midwife
A midwife is a woman who attends Midwifery Education recognized by the
Government who has completed the education and passed the prescribed exam and
obtained a registered certificate as the main requirement to practice according to her
profession (Dekpes. RI, 1999:10). A midwife is a woman who has attended a three-
year education program and passed an examination with applicable requirements.
(Kepmenkes. RI. No. 900/Menkes/SK/VII/2002)

2. Midwife's Duties
In carrying out midwifery practice, midwives have various duties, including the
following:
Provide guidance, care and advice to adolescents, pregnant women with high
risk, mothers giving birth, postpartum mothers, breastfeeding mothers and mothers
during climacterium and menopause.
1) Help mothers who give birth and provide care for babies and school children.
2) Providing family planning services in order to create a healthy and prosperous small
family.
3) Carry out prevention and detection measures for infants and pre-school children.
4) Provide preventive measures and detection of the condition of mothers and children
under five whose health is impaired, as well as providing medical assistance as first
aid before further medical action is taken.
5) Guiding and training prospective midwives, shamans and health cadres within the
scope of midwifery services.
6) Reviewing midwifery service and care activities carried out for improvement and
improvement.
7) Motivate and mobilize the community, especially women, in order to realize the
health and welfare of midwives.

3. Roles and Functions of Midwives


In carrying out their duties, midwives have the following roles:
1) Role as implementer
Midwives have categories of tasks, namely:
(1) Independent task
Midwife independent task are :
a) Establish midwifery management for each care provided
b) Provide basic premarital services by involving them as clients
c) Provide midwifery care to clients during normal pregnancy
d) Provide midwifery care to clients during labor by involving clients/families
e) Provide midwifery care for newborns
f) Provide midwifery care to women of childbearing age who need family
planning services
g) Provide midwifery care for women with reproductive system disorders and
women in the climatological period and menopause
h) Provide midwifery care for infants and toddlers by involving the family
(2) Collaborative tasks
Midwife collaboration tasks are:
a) Implement midwifery management in every midwifery care according to the
collaboration function by involving clients and families
b) Provide midwifery care to high-risk mothers and first aid in emergencies that
require collaborative action
c) Provide midwifery care to mothers during work with high risk and emergency
situations that require first aid with collaborative action involving clients and
families
d) Provide midwifery care to mothers in the puerperium with high risk and first
aid in emergency situations that require collaborative action with clients and
families
e) Provide midwifery care for newborns with high risk and first aid in
emergencies that require collaborative action with clients and families
f) Provide midwifery care for toddlers with high risk and first aid in
emergencies that require collaborative action with clients and families
(3) Dependency/refer assignment.
Dependency tasks (referring to) midwives are:
a) Applying midwifery care needs in accordance with the function of client and
family involvement
b) Provide midwifery care through consultation and referral in cases of high-risk
pregnancies and emergencies
c) Provide midwifery care through consultation and referral during labor with
certain complications by involving the client and family
d) Provide midwifery care through consultation and referral to mothers in the
postpartum period accompanied by certain complications and emergencies by
involving clients and families
e) Provide midwifery care for newborns with certain disorders and emergencies
that require consultation and referral by involving the family
f) Provide midwifery care for children under five with certain disorders and
emergencies that require consultation and referral by involving
clients/families
2) Role as manager
(1) Develop public health services, especially midwifery services for individuals,
families, special groups and communities in the work area by involving the
community / clients.

(2) Participate in teams to implement health programs and other sector programs in
their working areas by increasing the capacity of traditional birth attendants,
health cadres and other health workers under their guidance in their working
areas.
3) Role as educator
(1) Provide and provide health education to individuals, families, groups and
communities regarding the prevention of public health problems, especially those
related to maternal, child and family planning health.
(2) Train and guide cadres and traditional healers, including midwives and nursing
students in their area or workplace.
4) Program as researcher/investigator
Conducting investigations or applied research in the field of health both
independently and in groups.
