Sandayong, Tuburan, Cebu, after having been duly sworn to in accordance with law, do hereby
depose and state that—

1. I am in possession of a Diploma given to me by CHNS-Luna Extension High School;

2. I kept the same inside my cabinet in the house for safekeeping puroses; however,
when I was in Mandaue City, my rented room was razed by a fire and my Diploma
was burned;

3. I am declaring the same to have been lost for all intents and purposes;

4. I am executing this affidavit in order to attest the truthfulness of the foregoing and for
whatever legal purpose this may serve.
