Letter of Request – Contribution to CVSKL Foundation

Regarding the matter, CVSKL Foundation has contacted Yayasan JCORP to inquire about potential
contribution to CVSKL Foundation. As requested, we are writing this letter to introduce CVSKL
Foundation and to request for contribution from Yayasan JCORP.

First of all, we would like to share the background of the foundation with you. CVSKL Foundation is
a non-profit charitable foundation established to support medical advancement and research
initiatives, raise awareness, and educate general public on cardiothoracic and vascular diseases as well
as to provide medical fees support for low-income group (B40) patients. CVSKL Foundation
successfully obtained tax exempt status from Inland Revenue Board (IRB) under subsection 44(6) of
Income Tax Act 1967 starting 1st Aug 2022.

Being a non-profit charitable foundation, sustainable funding is crucial for CVSKL Foundation to
maintain its operation and to continue contributing to society, especially to provide medical fees
support to B40 patients. Hence, CVSKL Foundation would like to invite you to contribute to the
foundation. The contribution can be in the form of bank transfer or cheque. Contribution details are as
such:
