AFFIDAVIT OF CONSENT

I, RODOLFO ROSARIO, of legal age, widower, Filipino, with residence at

Brgy. Camagsingalan, Sual, Pangasinan, after having been sworn to in accordance with
law, hereby depose and state that:

1. I am the registered owner of a parcel of land situated in Brgy. Camagsingalan, Sual,

Pangasinan with an area of Two Thousand Eight Hundred Forty Nine square meters
which I and my late wife owned in common covered by Transfer Certificate of Title
no. 8347;

2. Due to my advanced age, I have difficulty remembering things so I tend to forget

where I place some of my important documents;

3. To secure the certificate of title, I am authorizing my eldest daughter Nelia Rosario

Mendigorin, to possess, keep and secure in her possession the said owners duplicate
copy of the said title and the owners duplicate of the tax declaration of the said
property;

4. No other person except my eldest daughter shall possess and keep said title and tax
declaration of the above mentioned property;

5. I am executing this Affidavit to attest to the truthfulness of the foregoing statements,

for all legal intents and purposes it may serve.
