I also declare that I have no other sources of income.

1 In consideration of HDB registering my application to buy the flat (hereinafter referred to as the
RESALE FLAT) and to apply for the CPF Housing Grant and/or the Additional CPF Housing Grant, I
hereby confirm that my declared total gross monthly income in this form is true and correct, and
undertake to produce necessary income documents/Income Tax Assessment for the year in which my
flat application is made, upon request by HDB.

2 I hereby confirm also that I have not suppressed any information which may affect my eligibility
to buy a resale flat and to apply for the CPF Housing Grant and Additional CPF Housing Grant (if
applicable).

3 I am aware that, if my declared total gross monthly income in this form is untrue or incorrect,
HDB shall have the right to:

a) Cancel the application and/or compulsorily acquire the RESALE FLAT and recover the
CPF Housing Grant and/or the Additional CPF Housing Grant, where applicable, with
interest at such rate as the HDB may determine from time to time; or
b) Recover the CPF Housing Grant and/or the Additional CPF Housing Grant, where
applicable, with interest if the RESALE FLAT or any interest therein is sold, transferred,
assigned or otherwise disposed by the applicant or any mortgagee or any other person
on or before the expiration of the 5-year minimum occupation period.

4 I am aware that if I made any misrepresentation of a material fact (whether knowingly or


otherwise), misleading and/or false statement in relation to the application for the RESALE FLAT, HDB
may compulsorily acquire the RESALE FLAT, debar me from buying a flat* or taking over ownership of
a flat* by way of transfer, being included as essential occupier of a flat* or taking up an HDB housing
loan for a period of 5 years from the date of cancellation of the resale flat application or the date of
approval to compulsorily acquire the resale flat. Any existing application registered in the HDB’s rental
waiting list will also be cancelled. In addition to other remedies, the Housing & Development Act
(Chapter 129) provides that any person who makes a false statement is liable on conviction to a fine not
exceeding $5,000/- or to imprisonment for a term not exceeding 6 months or both.

5 I hereby agree and undertake that I shall at all times keep HDB indemnified in full against all
actions, proceedings, claims, demands, costs and expenses which may be brought against or incurred
by HDB arising from any of the above matters and in relation to this agreement.

6 I am aware that the conditions in this undertaking form part of the Terms and Conditions for
Purchase of a resale flat which are subject to the prevailing policies, rules and regulations which HDB
may change from time to time. HDB has the right to vary the Terms and Conditions for Purchase of a
resale flat at any time.
