To keep patients from being addicted to certain kinds of pain medications, doctors
would prescribe an alternate dosage of morphine and plain normal saline solution which
was called a Placebo.
Nurse Lucky is a newly hired registered nurse in the surgical unit of the hospital who was
hired because of his brother being the nurse supervisor of the said hospital.
The senior nurse told Nurse Lucky that pain medications are not given liberally because
of their addictive properties
As nurse Lucky was cleaning the nurses’ station, he found vials of pain relievers and IV
antibiotics under the sink.
One day, a patient’s relative approached nurse Lucky and asked him about the
supervisor, who then requested if he can have a refund for the three vials of medicine
that he bought from his brother at half the price
Nurse Lucky asked his senior nurse about what he found under the sink, but his senior
nurse only said to just pretend not to see it since everybody is doing it anyway
As nurse Lucky was investigating, he found out that some nurses do not administer
expensive medications but rather they administer a placebo even though it was not
ordered by the physician.
They charge the patient with the medication even though it is not administered to them
and it was also sold to other patients who need it at half the price. The money was then
divided to those who are involved

The problem is whether to keep silent and join the group in order to protect his brother and
earn more or to tell other people about what he found out which may lead to his brother’s
harm.
To keep silent and not tell anyone to preserve the relationship and the value of
gratitude towards his brother for helping him graduate and become a nurse.
To be quiet about the case but not joining the group in such practices to preserve the

Positive Effects:

They may change their actions in the future as they will contemplate the deadly consequence
towards the malicious act they committed.
Negative Effects:
His brother, together with the other nurses, may repeat the same spiteful actions in the future.
If the same actions will be committed, there’s a higher risk that other patients may be affected
or perhaps, die as well.
Over time, higher authorities may witness their wrongful actions which will lead to their
imprisonment.
Second Alternative: To tell the higher authorities of the hospital the truth

Lucky’s brother and the nurses involved will lose their jobs.
There’s a higher chance that Lucky’s brother with his co-nurses will be imprisoned.
Lucky’s brother might get really angry with him as he was betrayed by his brother whom he has
been supporting.
7. Make a Decision
After taking into consideration all of the information and facts that have been provided, as
someone who will decide as Lucky. We would tell the truth to the higher authorities of the
hospital to preserve the value of honesty, integrity, and justice in the workplace. The malicious
act, of administering a placebo without the physician’s order and not the required medications,
creates a greater problem both to the hospital and the patients. These things, along with the
additional charges placed by the nurses on the placebo they have been orchestrating, show so
much injustice to the patients and the hospital’s reputation as well.
Considering the welfare of Lucky’s brother and just letting things slip away because of indebted
gratitude, chances in the future of other nurses to be involved in the scheme and more patients
to be treated in this ruthless way would be in the line. Lucky’s brother, who’s been behind this
for a very long time, must also come to realize that his actions could create a big impact on the
health of the patients and the integrity of the hospital.
The reason above is founded on Kant’s Duty Ethics where “Morality is a Duty.” As in the
preceding situation, Lucky will consider speaking the truth not because of any external pressure
(considering his indebted gratitude to his brother) but because it is his moral obligation to do so
for the sake of the greater good. We must remember that it’s our duty to do good, not for the
reason that we have been ordered to, or we want happiness, or because we’re forced to do so,
but it is for the sole reason of performing our obligation. As what Kant said, “This duty, prior to
all experience, is contained as a duty in general in the idea of a reason that determines the
will...”
