Re: Visa Issuance

Dear Sir/Madam,
I am taking this opportunity to write to you regarding our student visa and our intention to return
back to campus.
I, Dwiyantoko Girindra Tedjokusumo (AUP ID#: 2051970), a third-year MLS student, have
been home in Indonesia since the beginning of the pandemic in March of 2020 and have been
taking online classes since. This semester the department of Medical Laboratory Science applied
for Limited In-Person Classes to CHED and the curriculum necessitates that I have to return
back to campus for my clinicals. As we are all aware, the country has been in lockdown since,
and as my student visa has expired in August of 2020, I have therefore not been able to travel
back to the Philippines due to the travel restrictions set by the Bureau of Immigration and that
the Philippine Embassy in Jakarta, Indonesia have put all visa issuance on hold. However, with a
letter from the university, the embassy would be able to process the documents needed for my
travel. I would be very grateful if you could provide a statement/letter that: I am still enrolled at
AUP as a student in the Medical Laboratory Science department of the College of Health and
that I need to go back to the campus to continue my studies through the Limited In-Person
Classes, so that I could apply for a tourist visa and an entry exemption document from the
Philippine Embassy, thus enabling me to enter the Philippines and return back to campus.
I thank you for all your kind attention and assistance to help me in obtaining a visa so that I can
return back to campus. May God bless you and our beloved university.

Sincerely,

Dwiyantoko Girindra Tedjokusumo
