    I, the undersigned, willfully and voluntarily give consent on the participation of my


    child ______________________________ in the Voluntary HIV Screening to be held
    on March 24, 2023 (Friday) at Sinogbuhan National High School, San Joaquin Iloilo.

    The rising cases of infection of Human Immunodeficiency Virus, especially


    affecting the young key population ages 15-24 years old, Rajah Community Center of
    FPOP-Iloilo powered by Love Yourself, Inc. is amplifying the advocacy in campaigning
    the education and prevention program for HIV. The community center does serve not
    just the Iloilo city but as well the Iloilo province.
