DESCRIPTION 
 
Effective immediately, Sylvania biax lamps previously used in our 2x2 general lighting fixtures shall no longer 
be reused (iPro #20519).  All other Sylvania lamps (LED spots, CFLs, Metal Halide, etc.) can be used and 
reordered.   
 
For disposition of the Sylvania biax lamps (iPro #20519), adhere to the following: 
 If you have unopened cases/shipping boxes of Sylvania biax lamps, the case can be returned for 
credit. 
o Contact the Sylvania account representative, Molly Cahill (contact information below), to 
arrange return of biax lamps for credit. 
 You will need to provide the purchase order (PO) number or claim number for the 
shipment, which is typically found on the exterior shipping label.   
 DO NOT open a case/shipping box in order to search for the PO or claim number, as 
that will render it ineligible for credit return. 
 If you have open cases/shipping boxes of Sylvania biax lamps, they should be recycled.

Additionally, if you have more than ten (10) unopened cases/shipping boxes of each type of Sylvania LED Spot 
lamps, quantities over the ten (10) can also be returned for credit.  Reach out to the Sylvania account 
representative, Molly Cahill (contact information below), to arrange for credit return of LED spot lamps. 

SPECIAL INSTRUCTIONS 
In general, if the need arises to return unopened cases/shipping boxes of Sylvania lamps for credit, reach out 
to Molly Cahill at molly.cahill@sylvania.com or 972‐742‐9519 to arrange returns for credit.
