Statement of Work for Professional Services

STATEMENT OF WORK

Supplier’s Name: Diversity Science

Scope of Work:
(Include a full description and purpose of the service. Identify deliverables, milestones, and dates.)
● Diversity Science will design and develop evidence-based antiracism and implicit bias training eLearning
course for Minnesota hospitals and birthing centers to provide to their perinatal care staff. Diversity Science
will work collaboratively with the Center for Antiracism Research for Health Equity to:
o Identify the necessary modifications to existing eLearning course modules created for the California
Dignity in Pregnancy and Childbirth Act based on the Minnesota context. Develop a new project
management plan given the shortened project timeline. Hold a project kick-off call with the CARHE
team.
o Adapt the eLearning course modules created for the California Dignity in Pregnancy and Childbirth Act
based on the Minnesota context and feedback from community stakeholders and report on project
progress to the CARHE team.
o Deliver SCORM-compliant course modules, including pre/post-testing and certificate generation, in
compliance with the state of Minnesota accessibility policies and standards.
● Deliverable Deadlines:
1. Project kick-off call: August 15, 2022
2. Progress report call: September 15, 2022
3. Audio recording: October 20, 2022
4. SCORM-compliant files for upload into learning management systems: December 16, 2022

Payment Schedule:
● Payments will be made for work completed, net 30 days, according to the invoicing schedule below.
● 25% of the total contract price will be invoiced at each deliverable due date as follows:
o August 15, 2022
o September 15, 2022
o October 20, 2022
o December 16, 2022
